Project: Promoting Health Care Quality  Section 3:
Quality Measurement and Assessment  Measuring and assessing quality provides a foundation for identifying needs for improvement, developing effective interventions, and then monitoring progress toward desired results.   In the Sadeghi, Barzi, Mikhail, and Shabot text, the authors assert that an effort to improve quality “calls for setting outcome and performance targets (goals and expectations) beginning with the end in mind, and then formulating initiatives and actions (processes) that can serve as a bridge to take the organization from its current performance to its desired performance” (p. 182).   Having this “end in mind” can provide justification for dedicating valuable resources toward the effort, and help to lay a clear, concrete path that takes the organization from uncovering gaps between current performance and evidence-based targets to actually achieving those goals.  In this section of the Course Project, you focus on what you would measure—and how you would measure it—relative to your previously identified quality improvement issue. Note: This section of the Course Project will serve as the Portfolio Assignment for this course.       Course Project: Section 3: Quality Measurement and Assessment: write a 3-5 page paper to introduce your quality improvement plan. For more details on this project, see the Week 7 Project area for specifics to include for this paper.     I have attached my paper from week 4 for the continuation of the project.     To prepare: •Review the Learning Resources. Also conduct a search of the literature related to your selected quality improvement issue and complete the Evidence Matrix (provided in this week’s Learning Resources).   •Evaluate the evidence presented in the research literature and the quality standards proposed by various organizations, including the IOM’s six aims for quality improvement.   •Based on this review and your knowledge of the organization that you have selected, determine appropriate measures and indicators of performance related to your identified quality improvement issue.   •Consider how you could gather and use historical evidence of the organization’s performance related to the quality improvement issue. Also consider methods for gathering and assessing current quantitative and qualitative data, including those currently in use by the organization (e.g., chart audit data, staff surveys, quality indicators that are monitored, observations). If possible, examine actual data to assess your selected organization’s performance related to the issue.   •Think about what, specifically, you would like to achieve related to this issue. Establish realistic, evidence-based quality performance targets.   •Analyze gap(s) between the organization’s current performance and the performance targets.   •As you do this, you may wish to create a graphic organizer/alignment chart for your own use that illustrates:   •The measures and indicators that you would evaluate   •Current and historical data related to this quality issue—either actual data or methods for how you would collect and analyze the data   •Methods for collecting and analyzing data in the future (including when you would do this)   •Realistic, evidence-based targets

XXXX Committee Meeting
A committee meeting is defined as, “A small group of people chosen to represent a larger organization and either make decisions or collect information for it (Committee, n.d.)”. Committees are an important factor for medical facilities all over the world. This paper discusses the role of the collaborative practice committee at Aspirus Wausau Hospital, the employees attending the meeting, the observation of interactions between them, and shared governance.
Functions of the Committee and Role of Attendees
Committees consist of employee participation in decision-making and a team work approach to decrease errors (Amirabadi, Maleki, Salehi, & Watankhah, 2016). The collaborative practice committee at XXXX Hospital meets on the first Wednesday of each month to discuss topics with staff on how to improve the unit. The function of the committee is to present topics such as falls, staff turnover, patient satisfaction, medication errors, and new information. Members of the unit attending the meeting have the ability to voice their opinions, questions, and concerns on how to go about changing and improving the unit. If needed, attendees will vote on new practices on the unit.
Personal Observation of Interactions
At the collaborative practice meeting, there were four main speakers, one manager, the clinical educator, the director, two respiratory therapist representatives, and over twenty floor staff including nurses, monitor tech, certified nursing assistants, and health unit coordinators. The collaborative practice president and vice president presented on the main scheduled topics and asked for audience feedback. Multiple staff joined in the conversation, some presenting additional issues to address. Employees took turns asking questions and adding to the conversation. At the end of the meeting, the two collaborative practice chairs spoke of surveys, when the next meeting would be, and where the next meeting would be.
Decisions of Shared Governance
Shared governance is a model that was introduced to improve the nurse work environment, satisfaction, and retention. The model was created to implement core values and beliefs that the professional nursing practice embraces while achieving quality care (Anthony, 2004). In the collaborative practice meeting, staff were able to ask questions, input their thoughts, present concerns, listen, and add to the overall conversation. In assessing the meeting to determine if shared governance was a factor, this writer believe that it was a part of the meeting. The reason being is because nurses with different roles and experience met to converse about how to make the cardiac unit a better place not only for patients, but staff as well.
Conclusion
Committees are an important aspect in medical organizations. They assist in breaking down issues in meetings by presenting them to staff. The meetings then accumulate information from staff members that are concerned, have questions, or simply want to place input. Meetings at Aspirus proudly uses shared governance because it is a vital role in committee meetings to help nurses feel that they are heard.

Quantitative Study
Churchill M , et al. (n.d.).  – PubMed – NCBI. Retrieved from https://www.ncbi.nlm.nih.gov/pubmed/10218187. Using a therapy dog to alleviate the agitation and desocialization of people with Alzheimer’s disease. A Matched Case–Control Trial. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper.
Background of Study
According to the The Alzheimer’s Association there has been an  estimated number of  4 million individuals suffer from Alzheimer’s Disease, also known as AD, and by 2050, this static  will continue to grow to an estimated 14 million (Alzheimer’s Association, 1998).  These figures are an indication that the care for those affected by AD will place a burden on health resources with the burden only expected to grow over time.  To counter the expected burden, it is projected that medical care should be focused on addressing the neuropsychiatric symptoms that include anxiety, apathy, depression, and agitation. These symptoms are chronically present in all patients and only worsen over time. Previously, psychotropic medication was used to address these symptoms. Nonetheless, evaluations show that these medication have little effect on the symptoms, and no effect on the quality of life often times resulting in the patient being lethargic along with little interaction in life and or family members at all.   AD is a neurological disorder that presents as a slow, progressive disorder that over time will lead the individual to experience a decrease in cognitive function exacerbated by impairments in memory, reasoning, abstraction, and personality (Manor, 1991). Based on this awareness, pet therapy along with other alternative methods, such as music therapy is proposed as a psychosocial intervention that could achieve the desired goal of addressing the neuropsychiatric symptoms of AD and dementia while improving the quality of life. As such, the present study evaluates usefulness of proposed therapies as mentioned to show improved patient outcomes as well as quality of life.  Even though it has been considered that  AD is a disease that affects only the elderly, it has been diagnosed in younger people (Alzheimer’s Association, 1998).  The cause of AD, even in today’s time with advanced technology is still unknown.  Secondary to the effect of this disease on the normal brain activity can often times result in the patient experiencing late afternoon symptoms, referred to as Sundowners. Behavioral changes that are associated with sundown syndrome include restlessness and verbal behavior (Evans, 1987); confusion; aimless wandering; paranoia; agitation; and aggressive behavior, such as hitting, kicking, and biting directed toward staff, family, or other patients that will take place during evening hours. Agitated patients often display symptoms such as uncooperative routines, that results in difficulties for the staff caring for the patient. The tendencies of these aggressive behaviors may lead the patient to have feelings of seclusion, loneliness, and fear, resulting in increased aggressive behaviors. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper.
Methods of Study
An experimental and repeated measure design utilized that was used for this study. The subjects were recorded under two random ordered conditions both with and without the dog present, being aware that this research project was approved through the Institutional Review Board and the Institutional Animal Care and Use Committee. Eligibility to be involved in the study included a diagnosis of AD or a related disorder such as Dementia, residence in a special-care unit, display of agitated behavior in the evening hours, no allergies to dogs or bad experiences with them, consent from the legally authorized representative person/persons.
There were three extended care facilities with special-care units in the Midwest that consisted of individuals with AD or a related disorder. Each of these facilities held 20 to 35 residents on the unit, and each unit had a common area (lounge) where residents congregated for the day. Those studied consisted of twenty-eight individuals to include 21 women, 7 men.  Each of those involved in the study were also noted to be caucasian with a mean age of 83 and older, all with different levels of severity of the disease. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper.
Results of Study
The presence of the therapy dog showed nothing less than encouraging results that included verbalizations, smiles, looks, leans, and tactile contact. The importance of looks and smiles could be considered as an indication of happiness and interest, as well as the importance of touch displaying a nonverbal approach to communication. When considering this aspect, one must consider the importance of touch even in those that do not suffer from a disease.  Companionship, touch, and the feelings of affection is necessary in all elements of physical and mental health regardless of age, race, gender, or even those suffering from disease. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper.
Ethical Considerations
The study mentions that ethical approval for the research was sought from all affiliated institutions after extensive consultation throughout the research process.  The approval was based upon that the study showed no harm to those that were involved in the study and adequate actions had been undertaken to ensure that the participants as well as the animals were protected. In addition, it mentions that there was a requirement for informed consent that was signed by the participants to show that they were aware of the research intentions and consented to participate. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper.
Conclusion
There is much to be learned about the best treatment for those that suffer from such a sad and deteriorating disease known as Alzheimer’s and Dementia especially knowing that there is rarely early detection and no know cure.  Family members are often times found to be at a loss and only searching for ways to bring their loved ones back to who they used to be.  Even in our worse state, it is simply amazing how a simple thing such as touch, feelings of love and security can make all the difference in the world to a bad day.  This study showed that the presence of a therapy dog did improve socialization and decreased agitation in persons with AD or a related disorder during sundown hours. These changes, however, were found to be unrelated to the severity of the disease process.  With this being said, the results are valuable to nursing practitioners since they show a clear correlation between those that suffer from AD and dementia and that with the use of pet and other alternative methods such as music therapy are under review so that nurses will have evidence to support proposals for development of pet, music and possible other therapy programs. Alzheimer’s and Dementia – Research Critiques and PICOT Statement Final Draft Assignment Benchmark Essay Paper

Examples of ethical dilemmas in nursing essays
Scenario 1: Medical coding in a physician’s practice
Imagine you work in a high-pressure cardiology physician’s office and you are one of two medical coders. Your supervisor is very focused on the greatest reimbursement to satisfy revenue projections for the practice. As a result, you are asked to “up-code” billing. How can the pressure of acquiring the maximum repayment for services lead to manipulating or falsifying documentation?
Scenario 2: Administration of patient medications in the hospital setting
Imagine you are a new graduate nurse working nights on a busy medical unit. You just received a new patient who needs to be admitted to your unit and you just finished medicating a patient with a narcotic injection with a dose greater than ordered. Clearly understanding medication errors may lead to patient injury and even death, explain why a clinician may choose not to report the incident.
Scenario 3: Not hiring a qualified individual because of discrimination
Imagine you are a new human resources director in a nonprofit organization and have been pressured not to hire Middle Eastern candidates by the organization’s CEO. In the United States, discrimination against people based on their ethnicity, race, or cultural orientation is strictly forbidden under federal and state laws. Ethical discrimination may result in the breeding of ill feelings at work, as well as reduced productivity. To eliminate these ramifications, organizations need to put forth increased effort in curbing ethical discrimination in the employment sector. What are some interventions organizations can put in place to prevent discrimination?
Write a 150- to 350-word response for each ethical scenario. Include a title page with your name and your responses.
Format your assignment according to APA guidelines. Include a title page, an introduction, headings, in text citations, a conclusion and a separate reference page.
