NURSING – SPIRITUAL ASSESSMENT – PHI

DUE FRIDAY AT 9AM – BE SURE TO HAVE AN INTO AND CONCLUSION

Details:

This assignment requires you to interview one person and requires an analysis of your interview experience.

Part I: Interview

Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee’s experience as a patient, regardless of whom you choose to interview.

Review The Joint Commission resource found in topic materials, which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.

Your spiritual needs assessment survey must include a minimum of five questions that can be answered during the interview. During the interview, document the interviewee’s responses.

The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.

Part II: Analysis

Write a 500-750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

  1. What went well?
  2. Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
  3. How can this tool assist you in providing appropriate interventions to meet the needs of your patient?
  4. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.

Submit both the transcript of the interview and the analysis of your results. This should be submitted as one document. The interview transcript does not figure into the word count.

Assessment Introduction: The emphasis in this assessment is on your understanding of the ethical, legal and professional obligations of a registered nurse (RN), as outlined in the eight principles of the Code of Conduct (NCNZ, 2012). You will demonstrate that understanding through an exploration of, and reflection on, one interaction between a registered nurse and a patient/client you observed during a clinical placement. The interaction you select can be either positive or negative for the client/patient. Assignment Tasks: Task One: The interaction (300 words) Briefly summarise one interaction between a registered nurse and client/patient you observed during a clinical placement. You must ensure anonymity and confidentiality of all the parties and the clinical setting when developing your summary. The interaction you select can be either positive or negative for the client/patient but must be sufficiently complex to enable an indepth analysis of the ethical, legal and professional obligations of a registered nurse. Task Two: Evaluate the interaction against four (4) principles of the code of conduct. (1800 words) Identify which four of the eight principles in the code are MOST applicable. Then, For each principle, explain your understanding of the principle AND objectively evaluate the RN’s practice against this principle. Focus this discussion by choosing just one (1) or two (2) of the applicable standards. Support your discussion with appropriate literature. Task Three: The impact for professional nursing (300 words) Based on your overall evaluation of this interaction, briefly discuss how the practice of the registered nurse you observed contributes to, or detracts from, professional nursing within New Zealand. Discussion: Cardiovascular Disorders

Veins and arteries are vital elements of the cardiovascular system. They carry the blood supply through the body and are essential for proper function. Sometimes veins and arteries malfunction, resulting in cardiovascular disorders. Malfunctions of arteries and veins are similar to malfunctions of a water hose. Consider the structure and function of a hose. A tap releases water, which then travels through the hose and comes out the other end. If the hose has been dormant for several months, dirt and rusty particles might build up inside, resulting in a restricted flow of water. Similarly, buildup of plaque inside the coronary arteries restricts blood flow and leads to disorders such as coronary heart disease. This disease is one of the most common cardiovascular disorders, and according to the National Heart, Lung and Blood Institute (2011), is the leading cause of death for men and women in the United States. In this Discussion, you examine the pathophysiology of cardiovascular disorders such as coronary heart disease.

                                                             To Prepare

Review this week’s media presentation on alterations of cardiovascular functions, as well as Chapter 24 in the Huether and McCance text. Identify the pathophysiology of cardiovascular disorders.

Select one patient factor: genetics, gender, ethnicity, age, or behavior. Consider how the factor you selected might impact the pathophysiology of cardiovascular disorders.

Select one of the following alterations of cardiovascular disorders: peripheral arterial disease, myocardial infarction, coronary artery disease, congestive heart failure, or dysrhythmia. Think about how hypertension or dyslipidemia can lead to the alteration you selected.

Post a description of the pathophysiology of cardiovascular disorders, including how the factor you selected might impact the pathophysiology. Then, explain how hypertension or dyslipidemia can lead to the alteration you selected for patients with the factor you identified.

Here are weekly assignments to address for each week which will contribute to the Final Project: Each weekly assignment will between 250-350 words.

· Week 1 – Develop your network boundary based on the requirements provided, see Appendix A of the syllabus. Follow the assignment in Appendix A. You will need to draw the network boundary and provide a detailed network description of the network boundary.

· Week 2 – Describe the security and privacy requirements for the network boundary. This is a physician’s office, so please describe the HIPAA security and privacy requirements you need to follow for your network boundary. Use the HIPAA, HiTech, and Omnibus Laws to help you create HIPAA security and privacy requirements.

· Week 3 – We need to ensure the physician’s office is secure and the HIPAA data is protected. Read NIST SP 800-53 rev 4. How can this document help you ensure your physician’s office is secure? Out of the 18 control families, pick two control families and address the controls in complete sentences for your network boundary.

· Week 4 – We need to ensure the network boundary is hardened. Please review the DOD STIG for Oracle 12. Select 20 controls and address how the Oracle server has been hardened in the physician’s office.

· Week 5 – We are preparing for an audit of the system for HIPAA compliance. What are all of the documents we will need to have prepared for the upcoming audit? Please explain why each document is important. What scans should you run on the system, please describe the scan and on why systems the scan ought to be facilitated.

· Week 6 – The auditors have finished their assessment. In Appendix B, we have the findings from the audit. Please address in detail how each finding should be mitigated. Match up each control to the SP800-53 control family and control number.

· Week 7 – The physician’s office now wants to add tele-medicine to the functionality of their network. Explain in great detail, 500 words or more how this will impact the physician’s office and what we need to do from an information assurance perspective. Make sure you include change management in this discussion.

· Week 8 – Turn-in of FINAL PROJECT. Bring together your past 7 weeks of work. Add narrative transitions where appropriate and ensure you have addressed the instructor’s feedback provided each week. Conclude your project with a 500 word narrative explaining why information security is important in the Healthcare field. Cite Scripture to demonstrate your understanding of how faith integrates with the information technology and healthcare fields. Check to ensure that APA format has been used and you have at least 14 peer-reviewed references.

APPENDIX A

Information Assurance Project
In order to understand the practical impact of Information Assurance, we will work on a project over the next 8 weeks. One of the major requirements in information assurance is documentation and being able to articulate your understanding of a security requirement or control. Please design a network for a hypothetical physician’s office and provide a network description with the following:
1 Server with Scheduling software (pick one)
1 Server for billing (pick one)
1 Server with a data base for patient data – Oracle 12
1 Server for email – Microsoft Exchange Email
The office has 10 patient rooms with a desktop in each room running Windows 10 for the OS
The office is based on wireless networking with TCP/IP.
There are two doctors in this office.
This office has an Internet connection to the mother company.
The network boundary for this assignment is just this physician’s office.
In your network description please provide the following:
Describe the purpose of this network.
Describe the network and equipment, the servers and the software in place.
Describe the security you have in place.

APPENDIX B
The auditors have completed their assessment. The following are the findings determined during the audit. Please address in detail how each finding should be mitigated.

Identified   Vulnerability

Identify   the Matching Control in the SP 800-53 – Control Family and Control Number

What would   be the appropriate mitigations?

1. People can gain physical access to the physician’s office   without anyone checking ID.

2. The server room does not have a lock on the door.

3. There are default admin accounts with elevated privileges

4. The receptionist of the office provided the password to the   server via an inbound phone call.

5. There are unused open ports on all of the servers.

6. The scheduling software shows verbose code.

7. There is no encryption on the network. PHI/PII data is sent over   the wireless network in clear text.

8. The PHI/PII data on the database server resides on unencrypted   drives.

9. In an interview with the Nurse, she stated there is no training   for HIPAA Security or Privacy provided.

10. On the desktops, there are Microsoft vulnerabilities in the   Windows 10 OS which have not been patched.

11. The auditor watched an employee make changes to the Oracle   server without following change management.

Find one public site within your own State/county and you can carry this data over to the Unit 8 assignment. Go to the public link chosen and choose a health issue i.e. Children with Special Health Care Needs. Review the data and evaluate it. Report your findings in a thorough, well-organized manner. For example, if you live in Los Angeles county, California, Ask yourself these questions as you gather your data: What can you assume from this health assessment? Take into consideration the population demographics used in Unit 4. What information contained in the report could help you in planning interventions in community health? What information could be used in public policy planning? Format your paper consistent with APA 6th guidelines. Be sure to list your sources of information at the end of your assignment. You should have at least 3 sources.

Question Description

1. Long-Term Care in the U.S.

Determine the average daily cost of nursing home care in your state. Discuss the funding options (public and private) that exist to pay for long-term care. What policy and financing recommendations would you make to ensure that long-term care is available for everyone who needs the services in the U.S.? Please justify your answer.

2. Rationing of Healthcare Services

Healthcare services can be rationed by governmental rationing or by allowing those who can afford the services to purchase the services. If you were the chairperson of a medical ethics committee faced with the decision concerning which of the following patients would receive a heart transplant, what decision would you make and what justification would you give to the families of each of the patients for making that decision?

  • Patient 1: A seventy-six-year-old nonsmoker with good pulmonary and renal function is a good candidate for a heart transplant, and his life expectancy without the transplant is one month. The patient has the resources to pay cash for the transplant procedure and follow-up care.
  • Patient 2: A forty-six-year-old unemployed auto worker with a history of alcohol use and smoking suffered a heart attack and will die within one month if he does not receive a heart transplant. The patient does not have health insurance, nor does he have the resources to pay for the procedure.
