Ethical Considerations.

Last week, you explored practice problems at the local level. This week, we’ll take that knowledge a step further—you’ll have the opportunity to examine the protection of individuals who volunteer to participate in research studies. What ethical principles are researchers held to as they design a sampling plan that includes recruitment, informed consent, and protection of human subjects throughout the duration of the study, including the dissemination of research study findings? Why is a researcher’s ethical responsibility to human subjects relevant to your exploration of a practice problem? The Office for Human Research Protections (OHRP) provides leadership in the protection of the rights, welfare, and wellbeing of human subjects involved in research conducted or supported by the U.S. Department of Health and Human Services (HHS).  I’m sure you’re ready to find answers to these questions, so let’s get started!
Translation of a research-based intervention to practice involves a critical review of numerous published research studies to discern if individuals – research subjects – were treated according to the ethical values and principles as defined by the Code of Federal Regulations (CFR). Reflect upon your selected practice problem (lung cancer) and the eventual research-based intervention you select for your practice change project and address the following.

  • What are the potential benefits and harms related to your selected practice problem(lung cancer) when considering a research-based intervention for your practice change project?
  • Are there competing personal or professional values related to this research-based intervention that might impact the implementation of this intervention in your practice setting?
  • What types of objections might be raised? How will you explain your decision to key stakeholders to address these objections?
  • Utilize ethical considerations and theories relevant to translation science.

Instructions:
Use an APA style and a minimum of 200 words. Provide support from a minimum of at least three (3) scholarly sources. The scholarly source needs to be: 1) evidence-based, 2) scholarly in nature, 3) Sources should be no more than five years old (published within the last 5 years), and 4) an in-text citation. citations and references are included when information is summarized/synthesized and/or direct quotes are used, in which APA style standards apply.

Final Project, Part II: Evaluate a Quantitative Study

For Part II of your final project, you will comprehensively evaluate an article of your choice that reports on a quantitative study. (Remember, a quantitative study uses numbers as data.) Locate an article in the professional literature that addresses a topic you are interested in. Remember to use the Area of Interest interactive (Links to an external site.) to guide you in the selection process. The article you select should describe a quantitative study and should have all or most of the expected sections, either with their own headings or embedded in the article: Introduction, Literature Review, Methods, Results, and Discussion.
In writing your evaluation of the article, you will use Section 5.7 in your text. Do not simply tell what is in the article, but evaluate it using the questions in your text. Explain and justify your evaluation based on specific examples from the article.
Your paper should be 2100-2800 words in length (not counting the title, abstract, and reference pages) and comprehensively evaluate an article of a quantitative study. Use proper APA formatting and good writing and organization in your evaluation paper.
The following questions may help you in evaluating your articles, though you do not have to follow this format exactly.

  1. Evaluate the Introduction and Literature Review.
    • Do the researchers present an adequate rationale for conducting the study? Explain.
    • What is the significance of the study?  What difference will it make to the field?
    • Is the literature review thorough and comprehensive?
    • Do the researchers demonstrate any potential biases in the literature review?
    • Are all-important concepts clearly defined by the researchers?
    • Do the researchers clearly describe previous methods that are relevant to understanding the purpose for conducting this study?
  2. Evaluate the Purpose Statement and Hypothesis.
    • Does the article clearly state the purpose statement?
    • What is the purpose statement as expressed in the article?
    • Is the purpose statement clearly based on the argument developed in the literature review?
    • Are the variables of interest (i.e. independent and dependent) clearly identified in the purpose statement?
    • What type of hypothesis is presented (alternative nondirectional, alternative directional, or null hypothesis)?
    • Do the investigators provide a clear rationale for the direction of their hypothesis, based on the literature review?
    • Is the hypothesis testable and falsifiable?
    • Is the hypothesis stated with brevity and clarity?
  3. Evaluate the Methods Section.
    • What type of sampling method is used?  Is that appropriate?
    • Are relevant demographic characteristics of the sample clearly identified?
    • Do the methods of sample selection used by the researchers provide a good representative sample, based on the population?
    • Are there any apparent biases in selection of the sample?
    • Is the sample size large enough for the study proposed?
    • Is a particular quantitative research design used (e.g. true experimental, quasi-experimental, etc.)? If so, what is it?
    • Is the research design consistent with the purpose and hypothesis presented in the introduction?
    • Are the independent variables clearly defined so that they can be replicated using the information provided in the procedures section?
    • What are some of the potential threats to internal validity?  How are they threats?
    • What are some of the potential threats to external validity?  How are they threats?
    • Is there a clear and adequate description of the instrument (data collection measures) used?
    • What types of measures were used in the study (direct observation, behavioral measures, self-report, etc.)?
    • What are some of the potential problems or limitations with the types of measures used?
    • To what extent are the instruments standardized, and what types of standardized scoring were used?
    • Does the instrument appear to be appropriate for the sample?
    • Is the reliability of the instrument reported?  Is the reliability adequate?
    • What types of validity are reported for the instrument used?  Is the validity adequate?
  4. Evaluate the Results Section.
    • What descriptive statistics were used?  Do the researchers adequately describe the sample?
    • What types of inferential statistics were used? Are the inferential statistics used adequate to answer the research hypotheses or questions?
    • Is the analysis method clearly described?
  5. Evaluate the Discussion Section.
    • Do the researchers clearly restate the purpose and research questions?
    • Do the researchers clearly discuss the implications of the findings and how they relate to theories, other findings, and actual practice?
    • Do the researchers identify potential limitations of the study and the results?
    • Do the researchers identify possible directions for future research?
  6. Remaining Questions.

    Material in this paragraph should come from your research to questions 1 – 3 in Assignment #1. Be sure to introduce the information you will be sharing in the body of your essay.
  • Paragraph 2 – Body of essay
    This paragraph should cover the personal life of your composer. Using your responses to questions 4 – 11, create a quality overview of your composer’ life.
  • Paragraph 3 – Body of essay
    This paragraph will cover the musical works of your composer. Your responses to question 12 will give you the material to create this paragraph.
  • Paragraph 4 – Body of essay
    This paragraph will discuss the lasting contribution your composer made to music history and why they are remembered still today. Your response to question 13 will be the basis for this paragraph.

Capstone Project Milestone 1:

Practice Issue and Evidence Summary Guidelines

Purpose

Clear identification of the practice issue is the first step in evidence-based nursing. Next, the evidence is reviewed to determine the best intervention to change practice outcomes. Completion of the milestone will include identification of the practice issue using the ACE Star model of Knowledge Transformation and a review of the evidence that will support an intervention that will change outcomes. The evidence summary will be conducted through the ‘breaking down’ of a systematic review on your topic for your change project.

Course Outcomes

This assignment enables the student to meet the following course outcomes: 

• CO4: Develops and outlines a scientific, systematic decision-making process to integrate critical thinking with clinical judgment to assure safe and effective outcomes. (PO #4)

• CO8: Selects evidence for best practice when planning professional nursing care for individuals, families, aggregates, and communities. (PO #8)

Clinical question

“The thoughtful development of a well-structured EBP question is important because the question drives the strategies used to search for evidence”. (Dearholt & Dang, 2012, p.64). My question is: Will Congestive Heart Failure patients discharged from the hospital increase the chance of not being readmitted within 30 days with follow up care that includes post discharge telephone calls for support? 

P- (Patient, Population)-Congestive Heart Failure Patients

I-(Intervention)-Follow up phone call at 48 and 72 hours post discharge

C-(Comparison with current practice)-No follow up telephone calls for support.

O-(Desired outcome)-Reduce the number of Readmissions

In this class, we learn to evaluate issues in light of the reasoning on all sides prior to arriving at conclusions. We aim to evaluate the quality and quantity of evidence, striving to be as objective as we can about what is most likely to be true.
If you have not done so already, begin by choosing a topic from the Final Paper Options list (located in your online classroom) to use in your writing assignments in this course. The next step is to formulate a specific research question that is important regarding this topic. You may review The Research Process (Links to an external site.) resource for more information. So, if your topic is gun control, you would formulate a specific question, such as, “Are universal background checks effective at reducing violent crime in America?”
Once you have formulated your question, conduct research from non-scholarly sources on the internet (e.g., news articles, op-eds, etc.) that present substantive reasoning on each side of the issue.
Your task is to present and evaluate the reasoning from a non-scholarly source on each side of your issue. There is no need to take sides on the issue at this stage. In your analysis, strive to be as objective as possible, evaluating the reasoning from a neutral point of view. For an example of how to complete this paper, take a look at the Week 1 Example paper.
Your paper should include clearly labeled sections addressing the following elements:

  • Introduction (approximately 100 words)
    • Explain your topic.
    • State the specific question that you are addressing.
  • Presentation of an Argument
    • Describe the non-scholarly source (e.g., an op-ed, newspaper article, website, etc.) on one side of the issue.
      • Summarize the key points made (approximately 50 words).
    • Present what you see as the main argument from that source. Make sure to present your argument in standard form, with the premises listed above the conclusion. (approximately 100 words)
    • Evaluate the quality of the reasoning in this source (approximately 200 words)
      • In completing your evaluation, consider assessing how well the research supports the premises of the main argument and how strongly the reasoning supports the conclusion of that argument.
  • Presentation of an Argument on the Other Side of the Issue
    • Describe the non-scholarly source on the opposite side of the issue.
      • Summarize the key points made. (approximately 50 words)
