Care Plan
40 Points Due Date April 19, 2017 Directions:
Use the following information to create a care plan that identifies resources to assist G.T. and her daughter. G.T. is a 69 year old Hispanic woman. She was widowed five years ago and lives alone. Her son and daughter live near-by. She emigrated from Mexico in her teens, and married a man of eastern Indian decent that she met in an English speaking class. G.T. has a high school diploma, and worked for many years as an administrative assistant to an insurance executive. Her mother followed a Curandero as her primary health care provider when G.T. was growing up in Mexico. G.T. follows some western health care practices at the insistence of her daughter, but leans towards more cultural beliefs and sees a Curandero along with a physician. G.T. loves to cook and garden, and makes many traditional foods. She is retired now, has a cat and is very involved in her church. G.T. has a medical history of Diabetes Mellitus Type II, which is not well controlled, hyperlipidemia, hypertension, and gout. G.T. has developed incontinence, and she fell and fracture her right hip one month ago. She was hospitalized, and has been discharged to her daughter’s home. A home health nurse has been assigned to follow G.T., and is making her third visit to the home. G.T. is still incontinent, and experiencing constipation because she won’t drink fluids due to the incontinence. Her daughter, D.M., is caring for G.T., and tells the nurse: “I just don’t know what to do, I can’t take it anymore.” When asked about their relationship, her daughter states “I don’t mean to, but I yell at her sometimes, and then I feel so awful”. She also shares that she has to wash her mother’s linen every day, and asks “Can we put in a foley catheter, I am struggling with laundry”. Her daughter goes on to explain that her mother refuses to eat what the rest of the family eats, and that she is having trouble keeping up with making separate meals since her mother “hardly eats anyway”. When asked about her mother’s activity level her daughter says “I think she lays in bed most of the day, and stays up all night watching movies, I think she is in pain, but she denies it. Then I catch her taking medicine, and am worried if she is taking it at the right time”. Care Plan Organization and Formatting Use the provided Care Plan Template. Use professional language, free of spelling and grammar errors. Include at least twp peer-reviewed or evidence based resource to support the care plan. The Care Plan must be well organized, following the Nursing Process with a logical flow and accurate content that clearly relates to the topic. Follow APA guidelines to cite sources and provide a reference page. Care Plan includes the following: Data Collection and Organization Include subjective and objective data to support each nursing diagnosis. Be thorough and detailed, data should be clustered to each nursing diagnosis, and be sure to demonstrate cultural awareness in your assessments. Use your imagination to fill in gaps so that a clear picture is provided. Three priority nursing diagnoses: Each nursing diagnosis is written following the correct format Supports attention to immediate needs of the patient and her family At least one addresses the patients culture At least one addresses caregiver stress One short term and one long term SMART Goal for each nursing diagnosis: Each goal follows the SMART format Each goal demonstrates cultural awareness by aligning with the patient’s health beliefs, traditions, and practices. Three evidence-based nursing interventions for each goal List the nursing interventions in order of priority. Each nursing intervention demonstrates cultural awareness by aligning with the patient’s health beliefs, traditions, and practices. Provide a rationale for each nursing intervention supported with a peer-reviewed or evidence based resource. Goal Evaluation Provide an accurate and realistic description of how you would evaluate each goal. Provide an alternate outcome or intervention for each goal.

Question Description

Not so many years ago, nurses wore white uniforms and stiff white caps. They gave up their chairs for physicians at the nurse’s station. They cared for patients who stayed in the hospital for days or weeks to recuperate from surgery. They received a technical education, often in a diploma program, and carried out task-based nursing duties. This picture is as antiquated now as today’s nursing model will be in 20 years. In this module, you studied the role nursing plays in healthcare delivery. Tasks: Conduct an interview with a nurse manager and discuss today’s nursing roles in the healthcare delivery system compared to roles 5, 10, 15, and 20 years ago. Include questions in the interview that address the responsibilities of a nurse manager and the role’s accountability. Include the interviewee’s thoughts on cost containment within the healthcare delivery system. Write a 2- to 3-page interview report. In your report, include the following: •Identification and description of today’s nursing roles in the healthcare delivery system compared to roles 5, 10, 15, and 20 years ago. •Explanation of the responsibilities of a nurse manager. •Description of the accountability associated with nurse manager. •Explanation of the interviewee’s thoughts on cost containment within the healthcare delivery system. Support your findings with examples and scholarly readings or articles .Apply APA 6th ed. standards to citation of sources.

Contemplate what you need to consider when motivating students to  work with a partner or a small group. Think about how your specific  instructions in class reflect your teaching philosophy. For example,  will you give learners direct instructions to do each step of the task  or will you give them some general ideas and let them create their  project?

Write instructions (no more than 3–4 steps) for one mini partner or  group activity about one basic concept that would be taught in a CACREP graduate counseling course.-  (See these guideliness and standards — https://www.cacrep.org/for-programs/2016-cacrep-standards/
Provide a rationale or theory to support  the choice of activity and ways in which this activity will promote  learner motivation.
List two challenges that could occur with this  activity and how you would monitor and adjust during class to  accommodate the different student issues and needs.
Use current (within 5–7 years) primary scholarly resources to support statements.

SWK-543: SOCIAL WELFARE AND POLICY
FINAL PROJECT *Note: Please plan ahead for the interview assignment in Workshop Five.
Policy Analysis
Details Workshop Two Policy Analysis—Section 1: Social Welfare Issue/Policy Selection (two pages)
Identify a social welfare issue of interest.
• Why did you choose this issue? If you feel comfortable responding, what personal stories can you share about it?
• Research facts and statistics about the issue—what do we know about it? • What are current perceptions of the issue?
Research various social welfare policies associated with the issue.
• Browse bills from the U.S. Congress at the GovTrack.us: Social Welfare website: https://www.govtrack.us/congress/bills/subjects/social_welfare/6328. You can narrow your focus by drilling down into five subcategories (disability assistance, national and community service, poverty and welfare assistance, social security and elderly assistance, and social work/volunteer service/charitable organizations).
• What past and current policies address the issue? (An option is to create a timeline to show that policies are incremental.)
Identify a specific current social welfare policy associated with the issue that you will research throughout the course.

• Explain how knowing more about this specific social welfare policy relates directly to the NASW Code of
Ethics.
Workshop Three Policy Analysis—Section 2: Historical Review (two pages) Using the specific current social welfare policy associated with the issue you selected in Workshop One, conduct a historical review:
• Discuss the historical development/evolution of the social policy (social issues/problems that led to the need for the policy to be developed, what has changed over time, etc.).
• Identify the implementation level of the policy (federal, state, or local). • Identify the agency, department, or commission involved in formulating or implementing this policy.
(Give background information on the agency’s mission, budget, organization, etc.) • Explain why the policy was developed and to what issues or problems it was responding (goals,
strategies, etc.). What are the main causes of the issue? • Discuss what political or social pressures shaped the policy’s framework. • Identify what the policy was intended to accomplish (original intent of the policy). • Describe how the policy attempts to address the social problem in question. • Discuss the ideology and values (the basis of or inherent in the policy) that underpin this social policy,
including any specific political influences. • Describe how the policy aims to address the three social justice values that drive choices in policy
formation: equity, equality, and adequacy.
Workshop Four
Policy Analysis—Section 3: Target Population (one page) Identify the demographics of the target population (statistical descriptors), including who benefits and under what conditions (eligibility criteria) and for how long (time, age limits).
• What benefits are allocated? How are these benefits delivered and financed? • Define who is affected and how they are affected.

Workshop Five Policy Analysis—Section 4: Implementation Realities—Primary and Secondary Sources (two pages)
Gather primary and secondary source information about the policy implementation realities. Compare and contrast the primary and secondary sources and synthesize the information.
Primary Sources Conduct an interview with a social worker who may have been affected by the policy. Ask him/her the questions, listed below, regarding analyzing the policy in light of its stated intent.
• Discuss the implications of the policy in regard to gender, race, and class. Does the policy “do” what it intended to? If not, why? If so, how well?
• What are current perceptions of the policy? • Analyze the policy’s strengths and limitations in addressing the social welfare issue. • Describe current issues with the impacted population as well as the policy. • Describe barriers or potential barriers to the effectiveness of the policy. • Discuss alternative approaches that might better embrace a social justice perspective, or affirmation
that the policy is working to attain its goal: What approaches or solutions might better embrace a social justice perspective? If the policy is working to attain its goal, discuss how and why.
• Discuss how this policy is working (or not) to advance social justice within society. • What is the existing institutional capacity for implementing this policy? What are the potential conflicts?
Secondary Sources Conduct research using the same questions that guided your primary interview. Using LexisNexis, research and analyze the policy in light of its stated intent.
Use the LexisNexis Guide (http://www2.indwes.edu/online/socialwork/searchingfornews.pdf) to help you find peer-reviewed sources. The OCLS LexisNexis will lead you to credible and reliable sources. You will be able to access news regarding your topic from numerous sources.
Answer the questions using your research from secondary sources:

• Discuss the implications of the policy in regard to gender, race, and class. Does the policy “do” what it
intended to?
Workshop Six Policy Analysis—Section 5: Recommendations (one page)
Propose solutions to increase social justice related to this issue/policy: • What are three possible solutions to this issue? • Which one would lead to the highest level of social justice? • Is it the solution you would choose to work toward, or would you choose one that achieves less social
justice but might be more easily adopted or implemented?
