USING POLICY TO STRENGHTEN HEALTH CARE SYSTEMS (HAR 679) • T assignment is designed to assess your learning over the course of this module. It provides you with an opportunity to demonstrate that you can identify issues facing a health system in a particular national or local context, critically evaluate policy options to address health system issues and explain ways in which health systems may be changed and improved through different policy measures. Brief: Imagine that you are working for a policy taskforce, which has been commissioned by, and is required to report to, the political Head of a Public Health Department/Ministry in a particular health system context – you can choose which health system context you want to focus on. You have been asked to write a 2500 word report which does the following: • Identifies and describes a current issue facing a health system in a particular context • Suggests two possible policy measures that could address the health system issue identified (here, you are asked to be clear about what each policy measure will involve, what particular aspects of the health system each policy measure would strengthen (i.e. the ‘building blocks’, particular relationships, the roles of actors) and also how different actors would be involved in each policy being proposed (i.e. the roles different actors would take on). • Critically evaluate these two possible policy measures in terms of how particular groups, communities or people may or not benefit from, and/or may or not support or resist that strengthening, and • On this basis, select one policy measure to be taken forward and briefly explain why you have made this choice.

For this assignment, you must locate a scholarly article in one of the databases found in the University Online Library and write a critique that is at least two pages in length. The article that you use should address an issue related to diversity, equality, and equity in a public service organization. Your critique must include the following items:

  1. Identify the premise of the article and significant points to support the premise.
  2. Evaluate the issues of diversity, equality, and equity that are found in the article.
  3. Do you agree or disagree with the author’s premise? Explain.

Your article critique must be two pages not including title and reference page and formatted according to APA guidelines, including references and in-text citations.

Initial Post Instructions
Social psychologist Irving Janis coined the term groupthink to describe how people often respond differently in a group context than they might if they were alone. Another interesting topic of sociological study is the area of deviance and social control. The textbook provides multiples examples from history where groupthink may have influenced people to participate in socially deviant behavior that they might have otherwise avoided.
For the initial post, respond to one of the following options, and label the beginning of your post indicating either Option 1 or Option 2:

  • Option 1: Share an example from your personal experience (of your own or of someone you know), or an example from history, where groupthink may have influenced people to behave or arrive at a decision in a way that many individual members privately might have avoided or thoughts unwise.
  • Option 2: Choose a form of deviance with which you are familiar (not necessarily something you have done, but something someone you know did). Examine why society views that behavior as deviant and whether perceptions of that behavior have changed over time. Determine and explain which theory of deviance you think works best for understanding the deviant behavior you have chosen to discuss. Remember to be respectful in light of potentially sensitive topics, and consider what you might be disclosing, as there is no guarantee of privacy.

For this assignment develop a scholarly paper that includes the following criteria: 2-3 pages

1. Select a relevant clinical practice based problem/topic.

2. Describe the current clinical practice based problem/topic and include

3. Search the literature for four (4) evidence-based interventions on the problem/topic of interest. The literature can be individual studies or systematic reviews. A systematic review counts as one (1) intervention. If using a systematic review, report on the review, not on the studies identified in the review. The studies must be within the discipline of nursing (not medicine). All of the studies MUST be intervention studies. Review what constitutes an intervention study.

  • identify the databases (e.g., CINAHL, Medline) and key terms used in initiating the collective search for all of the studies (not each study)

4. Write the PICOT question in sentence form using the following format. The question must reflect an issue or intervention within the discipline of nursing (not medicine). This means the question must be about something nurses have authority over, not something that is prescribed and/or carried out by physicians or another discipline. Interdisciplinary interventions are encouraged. An example of a PICOT Question and PICOT template can be found on Appendix A or the accompanying Melynk & Fineout-Overholt (2014) 3rd edition text online.

·         P = Population –who does this problem primarily effect (i.e. Nurses, patients, families, etc.)?

·         I = Intervention or issue interest –what is the proposed intervention identified in the literature as best practice?

·         C = Comparison intervention or comparison group –what is the current practice? Compare the best practice intervention identified in the literature with the current practice.

·         O= Outcome –how would you determine if the change/implementation has been effective?

·         T = Time –how long would the new change/intervention need to be in effect before you anticipate evaluating the outcome?

Constant change in the health care delivery system requires nurses to be flexible in their role. Flexibility is the key to success during periods of change. Professional nurses need to open themselves up to a variety of ideas in order to arrive at a perspective of health care that enhances the practice of nursing. Fundamental to this concept is the ability to work in teams. No single individual can know everything that is required for practice today, and so the team becomes the focus. The Institute of Medicine emphasizes that teams improve skills, communication, participation, and effectiveness and includes use of inter-professional teams as one of the key five core health care professions competencies. Teams play a critical role in ensuring integration and coordination of care for patients.

Part 1: To begin the EBP quality improvement project group members will select a problem within the group that is common to all students. This one-page topic proposal should include the problem that you are working on, what are you trying to improve? Why did your group select this problem? The goal for the practice improvement project should be to “tell the story” of your improvement project. This is the first page of your group final presentation paper. This assignment should be preparatory work for your practice improvement paper and class presentation (it must be submitted to faculty for approval).

Part 2: This is a formal 10-12 page paper on your EBP practice improvement project. This assignment requires the use of evidence based references. Proper APA format and citation is expected for this assignment (See directions and rubrics).
