Background

As technology and computers have made their way into many professions, nursing has been no exception, and have been used to facilitate nursing practice which in turn led to the invention of health informatics. Health informatics knowledge has become increasingly useful in improving nursing care and practice. For instance, competencies of nursing informatics enable nurses to improve the quality of patients’ care outcome. According to Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, (2014), the introduction of nursing informatics indicates that nurses would be integrated into the field of information and communications technology and thus, results in enhanced delivery of services. One of the topics related to nursing informatics the following paper focuses on is computerized nursing management systems. It is highlighted in detail as follows.

Significance

First, computerized nursing management system is the application of software and hardware to manage and improve nursing care and practice. For instance, in a community-health based facility, computerized nursing systems are used to ensure operational efficiency and treatment of patients. Looking at the benefits, it helps to manage patients’ personal information, and finances, leads to active nursing engagement and integration of handheld devices (Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, 2014). In reference to management of patients’ personal information, the use of computerized nursing systems leads to improved financial management. As a result, it enables health facilities to monitor accurately their debtors and thus, improve their revenue collection and use. Secondly, using computerized nursing systems to manage personal information enables nurses to understand the health risks that people face. Consequently, they can create awareness and use the statistical information to enhance the quality of health services provided (Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, 2014). In addition, there is an active involvement of nurses with adoption of computerized nursing systems. For example, they help a health organization to select a system which is compatible with all staff and offer improved services.

Opportunities in Studying Computerized Nursing Management System

Studying nursing informatics as a career and specializing in computerized nursing management system makes health professionals, especially nurses, attractive in the job market. For example, health organizations currently seek nurses with informatics knowledge because they are capable of better management of patient’s private information. It is predicted by the Bureau of Labor Statistics that employers seeking this qualified will increase twenty five percent by 2020. The main reason for this is due to the continued increase in demand for computerized systems needed to enhance efficiency, and minimize expenses incurred (Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, 2014). Also, it is intended to improve patient care quality and proper management of patients’ personal information in the field of nursing.

Challenges & Benefits

However, with expected increase in job opportunities for nurses with knowledge in computerized nursing management systems, the current set of nurses are reluctant to be integrated into the field of information and communications technology. It means that by 2020, the job market’s demand for nurses with necessary health informatics knowledge might be unmet (Darvish, Bahramnezhad, Keyhanian, & Navidhamidi, 2014). Therefore, health institutions are encouraged to incorporate health informatics into the current curriculum to enable future nurses to be well equipped with computerized nursing management skills. It does not matter if the client has a backdated service order, the provider will not receive payment if there is not a service order in place. For example, if the client has a service plan dated for one-year, and the client goes into a substance abuse comprehensive outpatient treatment program a day after the year expired. Medicaid would not cover the outpatient treatment program until the client receives another service order. Still, would not be able to include any of the time before the service order date. Amend – the policy should have a clause reporting that “if” the client has received the same needed treatment within the one-year service order, then Medicaid should honor 72 hours of service. The 72 hour treatment time limit will give the professionals needed time to place another order while the client is being treated.  Without the funding for treatment, the client will be turned away –not receiving help. In the 72 hour time limit the client will receive value-based service while awaiting the decision of Medicaid approval.  In this amendment, there will be the inclusion of a specific department that handles renewal for Medicaid Service orders for the Medicaid and Health Choice Clinical Coverage – Enhanced Mental Health and Substance Abuse Services Department. Jessica is a 32 y/old math teacher who presents to the ER with a friend for evaluation of sudden decrease of vision in the left eye. She denies any trauma or injury. It started this morning when she woke up and has progressively worsened over the past few hours. She had some blurring of her vision 1 month ago and thinks that may have been related to getting overheated, since it improved when she was able to get in a cool, air-conditioned environment. She has some pain if she tries to move her eye, but none when she just rests. She is also unable to determine colors. She denies tearing or redness or exposure to any chemicals. Nothing has made it better or worse.

She is normally healthy. She had chickenpox at age 10 and a tonsillectomy/adenoidectomy at age 11. She has no medical problems. She has never been hospitalized. She has four children, all spontaneous vaginal deliveries.  She completed a bachelor’s degree in mathematics and a master’s degree in education. She quit smoking 10 years ago (two packs daily for 5 years); she drinks an occasional wine cooler, and she denies illicit drug use. Her father has a coronary artery disease (he had a stent placed at age 67) and a mother with hypertension.

She denies fever, chills, night sweats, weight loss, fatigue, headache, changes in hearing, sore throat, nasal or sinus congestion, neck pain or stiffness, chest pain or palpitations, shortness of breath or cough, abdominal pain, diarrhea, constipation, dysuria, vaginal discharge, swelling in the legs, polyuria, polydipsia, and polyphagia.

  3. What sorts of values and considerations would each religion focus on in deliberating about whether or not George should opt for euthanasia?

Case Study: End of Life Decisions

George is a successful attorney in his mid-fifties. He is also a legal scholar, holding a teaching post at the local university law school in Oregon. George is also actively involved in his teenage son’s basketball league, coaching regularly for their team. Recently, George has experienced muscle weakness and unresponsive muscle coordination. He was forced to seek medical attention after he fell and injured his hip. After an examination at the local hospital following hisfall, the attending physician suspected that George may be showing early symptoms for ALS (amyotrophic lateral sclerosis), a degenerative disease affecting the nerve cells in the brain and spinal cord. The week following the initial examination, further testing revealed a positive diagnosis of ALS. ALS is progressive and gradually causes motor neuron deterioration and muscle atrophy to the point of complete muscle control loss. There is currently no cure for ALS, and the median life expectancy is between three and five years, though it is not uncommon for some to live 10 or more years. The progressive muscle atrophy and deterioration of motor neurons leads to the loss of the ability to speak, move, eat, and breathe. However, sight, touch, hearing, taste, and smell are not affected. Patients will be wheelchair bound and eventually need permanent ventilator support to assist with breathing. George and his family are devastated by the diagnosis. George knows that treatment options only attempt to slow down the degeneration, but the symptoms will eventually come. He will eventually be wheelchair bound, and be unable to move, eat, speak, or even breathe on his own. In contemplating his future life with ALS, George begins to dread the prospect of losing his mobility and even speech. He imagines his life in complete dependence upon others for basic everyday functions, and perceives the possibility of eventually degenerating to the point at which he is a prisoner in his own body. Would he be willing to undergo such torture, such loss of his own dignity and power? George thus begins inquiring about the possibility of voluntary euthanasia.
