As a result of the fragmented nature of the health care system, professionals in various specialty areas of medicine have developed their own unique sets of terminology to communicate within that specialty. In the past, limited attention has been given to codifying practices in order for them to be understood and utilized across disciplines or through different information technology systems. The implementation of a federally mandated electronic medical records system, therefore, poses a challenge to nursing professionals and others who must be prepared to utilize standardized codes for the new system. Why are coding standards important for promoting consistent, high-quality care?

According to Rutherford (2008, para. 15), “Improved communication with other nurses, health care professionals, and administrators of the institution in which nurses work is a key benefit of using a standardized nursing language.” In this Discussion you consider the reasoning behind and the value of standardized codification.

To prepare

  • Review the information in Nursing Informatics: Scope and Standards of Practice. Determine which set of terminologies are appropriate for your specialty or area of expertise.
  • Reflect on the importance of continuity in terminology and coding systems.
  • In the article, “Standardized Nursing Language: What Does It Mean for Nursing Practice?” the author recounts a visit to a local hospital to view its implementation of a new coding system. One of the nurses commented to her, “We document our care using standardized nursing languages but we don’t fully understand why we do” (Rutherford, 2008, para. 1). Consider how you would inform this nurse (and others like her) of the importance of standardized nursing terminologies.
  • Reflect on the value of using a standard language in nursing practice. Consider if standardization can be limited to a specialty area or if one standard language is needed across all nursing practice. Then, identify examples of standardization in your own specialty or area of expertise. Conduct additional research using the Walden Library that supports your thoughts on standardization of nursing terminology.

Any discussion regarding healthcare for individuals age 65 and older must include Medicare. Medicare is the federally-funded healthcare program for individuals age 65 and older. Medicare has been at the center of many healthcare debates and its vitality is a major concern for Medicare beneficiaries.Another prevailing issue that may affect older adults is long term care services especially those services designed for older adults that require more support and assistance in such things as daily functioning (e.g. grooming, bathing, dressing, dining) and/or medical care.) Some common terms that are used to describe these services are aging-in-place and continuum-of-care.Some years ago, the term nursing home was changed to skilled nursing facility, in part due to the negative connotation of the name nursing home. Skilled nursing facilities (SNFs) are primarily reimbursed (paid) through Medicare and Medicaid (a government-funded program whose recipients are determined on the basis of financial need and other requirements).

  • Research some of the issues regarding Medicare. Explain how these issues have impacted older adults from a social and psychological perspective. Describe how older adults are dealing with perceived turmoil regarding Medicare.
  • Research skilled nursing facilities in your state or region. How do SNFs provide the needed psychosocial level of support for their residents? One of the major themes of this course was the aging of the US population. By 2030, all of the baby boomers (those born between 1946 and 1965) will have reached the age of 65. The aging population will also present challenges for many industries, especially healthcare. Advances in technology, particularly in medical technology, are likely to provide solutions for those challenges.

  • Based on what you have learned in this course, what are the most important issues facing older adults between now and the year 2030? Identify some specific areas which are being addressed now, in order to meet the challenges of a larger, older population in the future.

Examine the burden of disease in the United States to provide important information on which parameter is to base decisions on public health priorities.

To do this, we will utilize mortality data for the United States. In the first part of this assignment, you will download and examine mortality data for your home state.

Go to USA Causes of Death by Age and Gender

  • Choose your home state under the Choose State option (panel on left hand side)
  • Select BOTH under the Choose gender option in the middle panel.
  • Scroll down to the bottom of the page, and read the fine print to learn for which year the mortality data have been tabulated.
  • Copy and paste the relevant mortality data into Excel.
    • Drag your mouse over all of the Cause of Death rows, (50 rows), right click, and select Copy,
    • Open Excel and paste your selection into Excel. You should have a spreadsheet with 50 row and 19 columns (Columns A-S).

For the first part of the assignment, you will prepare a histogram of the leading causes of death (regardless of age) in your state. Follow the steps below in order to prepare your histogram:

  • Sort the Data
    • The numbers of deaths, all ages, are given in Column C.
    • Select all the columns.
    • Then, select Data>Sort>Sort by Column C, Values, largest to smallest. (Make sure that my data has headers is not selected.
    • You now have the leading causes of death in your state in Column A (cause) and Column C (frequency).
  • If you already know how to draw a histogram in Excel, proceed to do so with Columns A and C, making sure to truncate the data to the 30 leading causes.
  • If you do not know how to draw a histogram in Excel, here’s one method:
    • Choose the Chart Wizard, chart type column, chart sub-type clustered column (Step 1).
    • Click Next for Step 2.
    • At Step 2, Click the Series button, which will open a new box.
    • Click Add under Series,
      • Enter Causes of Death in the Name box;
      • Clear the Values box, then
      • Drag your mouse over the 30 largest frequencies for the Values; and,
      • Drag your mouse over the first 30 causes of death (Column A) for the Category (X) axis labels box.
  • Click Next, and you’ll be brought to the Chart Options box.
  • Add a suitable title (e.g., Leading Causes of Death, 2010, your state)
  • Label the Y axis Frequency.
  • Click Next, and place the histogram in a new worksheet.

You now have a histogram with the leading causes of death for your state. This presents one picture of the burden of disease in your state, but it isn’t the only picture. We shall now look at a different metric: years of life lost due to each cause.
