Consequences of a Healthcare Organization not Involving Nurses

Nurses are the backbone of healthcare and when they are not involved in the design and decision-making processes of the Systems Development Life Cycle the results can be catastrophic. According to the authors Powell-Cope & Nelson (2008), nurses are the frontline and chief users of electronic health record (EHR) systems, it only makes sense they’d have a major say in EHR design and upgrades. Unfortunately, however, in many facilities, EHR design is left primarily to IT and only minor input is taken into consideration from the nursing staff. When subject matter experts, such as nurses, don’t have autonomy and responsibility within the design process, implementing and utilizing an EHR can take longer because providers are distanced from the outcomes (Powell-Cope & Nelson, 2008).

Inclusion of Nurses in EHR Design

When nurses are included in technology design, it enables and enhances safety (Hamer & Cipriano, 2013). A study was done in 2009 on early nursing involvement during the implementation of a Bar Code Medication Administration (BCMA) system. The authors of this study describe how nurses participated in the early design, planning, implementation, and evaluation phases of the BCMA. The study found that the benefits of early nursing involvement in each phase of BCMA technology greatly outweigh the problems that can arise from early nursing involvement (Weckman & Jansen, 2009). This study found that in order to find success when implementing new technologies, it is essential that nurses be involved throughout all phases of the process. Comments and feedback from nurses provide the necessary clues that are needed to resolve underlying systemic issues and can offer possible resolutions.

My Personal Input

My current healthcare facility is changing its EHR system to Epic. They have selected a specific team of nurses and nurse informaticists that are currently part of their healthcare team, to design and adapt the Epic program to meet the institution’s requirements. My facility has named their adaptation of the Epic EHR to Elle. The entire healthcare team has been invited to monthly townhouse meetings which involve disclosing the most recent updates made to Elle and team members are also encouraged to provide input on any modifications they would like to add to Elle. As critical nurses, we are excited that we will finally have a charting system that downloads our vital signs electronically. Before Elle, we had to write our vital signs every 15 minutes on each of our two patients. It might not seem like a big deal, but writing vital signs for two patients can take up a considerable amount of time, especially when a patient is unstable and on multiple drips. If nurses were not involved in the EHR design, downloading vital signs might be something that was overlooked again.

CRS176: Nursing Care Plan- Primary Health Care Services- Nursing Report Writing Assignment

Task:

You are required to write a nursing care plan for a resident you are caring for on your placement.
1. You are to gather information on resident during clinical placement
2. You will need to ask permission from the clinical placement facility before you begin gather information about your chosen resident
3. No identifying information should be included in the case study
4. The resident should have dual diagnosis including but not limited to dementia and continence issues

Part A – Introduction
You must include an introduction to your resident that covers:
1. Their diagnoses and the impact this has had on the resident
2. Which theory of ageing you believe is most appropriate for this resident
3. Explain the physiology of ageing and how it relates to this resident’s disease process

Part B – Nursing care plan
Your nursing care plan must include:
1. Strategies and nursing interventions that can be used with people with dementia
2. Primary health care services for the older person
3. Strategies to respond to challenging behaviour
4. Complementary therapies
5. Meeting oral hygiene needs of a resident with dental prosthetics
6. Pain management for the older resident (including recognition of pain and pain triggers)

*There is no minimum number of nursing diagnosis or nursing interventions required for this assessment, however you should have no more than 5 actual and 4 potential nursing diagnosis (total of 9) for your resident. You need to meet the above requirements in order to be deemed satisfactory in this assessment. Please refer to the provide list of nursing diagnosis for a guide of actual or potential nursing diagnosis that you can use.

A registered nurse is writing a diagnosis for a 28-year-old male patient who is in traction due to multiple fractures from a motor vehicle accident. Which nursing actions are related to this step in the nursing process? Select all that apply.
The nurse uses the nursing interview to collect patient data.
The nurse analyzes data collected in the nursing assessment.
The nurse develops a care plan for the patient.
The nurse points out the patient’s strengths.
The nurse assesses the patient’s mental status.
The nurse identifies community resources to help The nurse analyzes data collected in the nursing assessment.
The nurse points out the patient’s strengths.
The nurse identifies community resources to help

Rationale: The purposes of diagnosing are to identify how an individual, group, or community responds to actual or potential health and life processes; identify factors that contribute to or cause health problems (etiologies); and identify resources or strengths the individual, group, or community can draw on to prevent or resolve problems. In the diagnosing step of the nursing process, the nurse interprets and analyzes data gathered from the nursing assessment, identifies patient strengths, and identifies resources the patient can use to resolve problems. The nurse assesses and collects patient data in the assessment step and develops a care plan in the planning step of the nursing process.A nurse is caring for an older adult patient who presents with labored respirations, productive cough, and fever. What would be appropriate nursing diagnoses for this patient? Select all that apply.
Bronchial pneumonia
Impaired gas exchange
Ineffective airway clearance
Potential complication: sepsis
Infection related to pneumonia
Risk for septic shock Impaired gas exchange
Ineffective airway clearance
Risk for septic shock

Rationale: Nursing diagnoses are actual or potential health problems that can be prevented or resolved by independent nursing interventions, such as impaired gas exchange, ineffective airway clearance, or risk for septic shock. Bronchial pneumonia and infection are medical diagnoses, and “potential complication: sepsis” is a collaborative problem. After assessing a patient who is recovering from a stroke in a rehabilitation facility, a nurse interprets and analyzes the patient data. Which of the four basic conclusions has the nurse reached when identifying the need to collect more data to confirm a diagnosis of situational low self-esteem?
No problem
Possible problem
Actual nursing diagnosis
Clinical problem other than nursing diagnosis Possible problem

Rationale: When a possible problem exists, such as situational low self-esteem related to effects of stroke, the nurse must collect more data to confirm or disprove the suspected problem. The conclusion “no problem” means no nursing response is indicated. When an actual problem exists, the nurse begins planning, implementing, and evaluating care to prevent, reduce, or resolve the problem. A clinical problem other than nursing diagnosis requires that the nurse consult with the appropriate health care professional to work collaboratively on the problem.A nurse assesses a patient and formulates the following nursing diagnosis: Risk for Impaired Skin Integrity related to prescribed bedrest as evidenced by reddened areas of skin on the heels and back. Which phrase represents the etiology of this diagnostic statement?
Risk for Impaired Skin Integrity
Related to prescribed bedrest
As evidenced by
As evidenced by reddened areas of skin on the heels and back Related to prescribed bedrest

Rationale: “Related to prescribed bedrest” is the etiology of the statement. The etiology identifies the contributing or causative factors of the problem. “Risk for Impaired Skin Integrity” is the problem, and “as evidenced by reddened areas of skin on the heels and back” are the defining characteristics of the problem.A nurse is counseling a 60-year-old female patient who refuses to look at or care for a new colostomy. She tells the nurse, “I don’t care what I look like anymore, I don’t even feel like washing my hair, let alone changing this bag.” The nurse diagnoses Altered Health Maintenance. This is an example of what type of problem?
Collaborative problem
Interdisciplinary problem
Medical problem
Nursing problem Nursing problem

Rationale: Nursing Problem, because it describes a problem that can be treated by nurses within the scope of independent nursing practice. Collaborative and interdisciplinary problems require a teamwork approach with other health care professionals to resolve the problem. A medical problem is a traumatic or disease condition validated by medical diagnostic studies. To determine the significance of a blood pressure reading of 148/100, it is first necessary for the nurse to:
Compare this reading to standards.
Check the taxonomy of nursing diagnoses for a pertinent label.
Check a medical text for the signs and symptoms of high blood pressure.
Consult with colleagues. Compare this reading to standards.

Rationale: A standard, or a norm, is a generally accepted rule, measure, pattern, or model to which data can be compared in the same class or category. For example, when determining the significance of a patient’s blood pressure reading, appropriate standards include normative values for the patient’s age group, race, and illness category. Deviation from an appropriate norm may be the basis for writing a diagnosis. When the initial nursing assessment revealed that a patient had not had a bowel movement for 2 days, the student nurse wrote the diagnostic label “constipation.” Which of the following comments is the nurse most likely to hear from the instructor?
“Hold on a minute . . . Nursing diagnoses should always be derived from clusters of significant data rather than from a single cue.”
“Job well done . . . you’ve identified this problem early and we can manage it before it becomes more acute.”
“Is this an actual or a possible diagnosis?”
“This is a medical, not a nursing problem.” “Hold on a minute . . . Nursing diagnoses should always be derived from clusters of significant data rather than from a single cue.”

Rationale: A data cluster is a grouping of patient data or cues that points to the existence of a patient health problem. Nursing diagnoses should always be derived from clusters of significant data rather than from a single cue. There may be a reason for the lack of a bowel movement for 2 days, or it might be this individual’s normal pattern.A nurse makes a clinical judgment that an African American male patient in a stressful job is more vulnerable to developing hypertension than White male patients in the same or similar situation. The nurse has formulated what type of nursing diagnosis?
Actual
Risk
Possible
Wellness Risk

Rationale: A clinical judgment that an individual, family, or community is more vulnerable to develop the problem than others in the same or similar situation is a Risk nursing diagnosis.A nurse is writing nursing diagnoses for patients in a psychiatrist’s office. Which nursing diagnoses are correctly written as two-part nursing diagnoses?
(1) Ineffective Coping related to inability to maintain marriage
(2) Defensive Coping related to loss of job and economic security
(3) Altered Thought Processes related to panic state
(4) Decisional Conflict related to placement of parent in a long-term care facility
Answers:
(1) and (2)
(3) and (4)
(1), (2), and (3)
All of the above All of the above

Rationale: Each of the four diagnoses is a correctly written two-part diagnostic statement that includes the problem or diagnostic label and the etiology or cause.A nurse writes nursing diagnoses for patients and their families visiting a community health clinic. Which nursing diagnoses are correctly written as three-part nursing diagnoses?
(1) Disabled Family Coping related to lack of knowledge about home care of child on ventilator
(2) Imbalanced Nutrition: Less Than Body Requirements related to inadequate caloric intake while striving to excel in gymnastics as evidenced by 20-pound weight loss since beginning the gymnastic program, and greatly less than ideal body weight when compared to standard height weight charts
(3) Need to learn how to care for child on ventilator at home related to unexpected discharge of daughter after 3-month hospital stay as evidenced by repeated comments “I cannot do this,” “I know I’ll harm her because I’m not a nurse,” and “I can’t do medical things”
(4) Spiritual Distress related to inability to accept diagnosis of terminal illness as evidenced by multiple comments such as “How could God do this to me?,” “I don’t deserve this,” “I don’t understand. I’ve tried to live my life well,” and “How could God make me suffer this way?”
(5) Caregiver Role Strain related to failure of home health aides to appropriately diagnose needs of family caregivers and initiate a plan to facilitate coping as evidenced by caregiver’s loss of weight and clinical depression
Answers:
(1) and (3)
(2) and (4)
(1), (2), and (3)
All of the above (2) and (4)

Rationale: (1) is a two-part diagnosis, (3) is written in terms of needs and not an unhealthy response, and (5) is a legally inadvisable statement.

Assignment 3: Long-Term Investment Decisions
Due Week 9 and worth 300 points

Assume that the low-calorie frozen, microwavable food company from Assignments 1 and 2 wants to expand, and has to make some long-term capital budgeting decisions. The company is currently facing increases in the costs of major ingredients.

Use the Internet and Strayer databases to research government policies and regulation.

Write a six to eight (6-8) page paper in which you:

  1. Outline a plan that managers in the low-calorie, frozen microwaveable food company could follow in anticipation of raising prices when selecting pricing strategies for making their products’ response to a change in price less elastic. Provide a rationale for your response.
  2. Examine the major effects that government policies have on production and employment. Predict the potential effects that government policies could have on your company.
  3. Determine whether or not government regulation to ensure fairness in the low-calorie, frozen microwavable food industry is needed. Cite the major reasons for government involvement in a market economy. Provide two (2) examples of government involvement in a similar market economy to support your response.
  4. Examine the major complexities that would arise under expansion via capital projects. Propose key actions that the company could take in order to prevent or address these complexities.
  5. Suggest the substantive manner in which the company could create a convergence between the interests of stockholders and managers. Indicate the most likely impact to profitability of such a convergence. Provide two (2) examples of instances that support your response.
  6. Use at least five (5) quality academic resources in this assignment. Note: Wikipedia does not qualify as an academic resource.

Your assignment must follow these formatting requirements:

  • Be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides; citations and references must follow APA or school-specific format. Check with your professor for any additional instructions.

Licensing and Professional Organizations

In Week 5, the Final Project will synthesize what you have discovered about the different advanced practice roles and scope of practice found in the master of nursing curriculum: NP, nurse educator, nurse informaticist, and nurse administrator. You will review all roles and then examine the specialty for which you were admitted, focusing on the scope of practice, core competencies, certification requirements, and legal aspects of practice for that specific role. This week you will focus on Licensing and Professional Organizations.

Based on the specialty area and role that you selected in W1 Assignment 4, in a 3- to 5-page paper (excluding the title page, references, and appendices) create a scenario or case study to illustrate the type of organization you would expect to work in as this type of nursing professional. Include in the scenario or case study:

  • List the type of organization.
  • List the type of and how many clients it serves.
  • Identify the professional fit for advanced nursing role.
  • Implement your new nursing role in the organization.
  • Identify a board of nursing in your state (specifically the Nurse Practice Act), which would support your role in this type of organization.

QUESTION

Nursing

Mark is a 15-year-old with complaint of acute left scrotal pain with nausea. The pain began approximately 6 hours ago as a dull ache and has gradually worsened to where he can no longer stand without doubling over. He is afebrile and in marked pain. Physical exam is negative except for elevation of the left testicle, diffuse scrotal edema, and the presence of a blue dot sign.

  • Review “Genitourinary Disorders” in the Burns et al. text.
  • Review and select one of the three provided case studies. Analyze the patient information.
  • Consider a differential diagnosis for the patient in the case study you selected. Think about the most likely diagnosis for the patient.
  • Think about a treatment and management plan for the patient. Be sure to consider appropriate dosages for any recommended pharmacologic and/or non-pharmacologic treatments.
  • Consider strategies for educating patients and families on the treatment and management of the genitourinary disorder.

The Role of Culture in Childrearing

Culture, representing what each individual has learned through his or her unique process of socialization, influences our theories of childrearing. Decisions about whether a 3-month-old should be allowed to “cry it out” or be picked up and soothed; when it is appropriate to begin toilet training; and where a child should sleep at night are all influenced, in part, by each individual’s beliefs. Understanding your own beliefs regarding childrearing and those of the families with whom you work is vital but only the beginning. Early childhood professionals must also be aware of how these beliefs may complement or contradict one another, and the related implications of both.
For this week’s Discussion, you will have the opportunity to explore the potential influence and impact of culture on childrearing practices, and discuss the relevance of developing “cultural competency” as a tool supporting infant/toddler mental health.
Pose the following questions to three to five people, keeping in mind there are no right or wrong answers. Your role is simply to listen and record their responses.

Where should a newborn sleep?
At what age should toilet training begin?
Should 3-month-old children be allowed to cry themselves to sleep at night?
What role does the extended family play in rearing children?
Based on what you have learned about the role of culture in childrearing practices, consider the following questions:

Based on your course readings and interviews, in what ways can culture influence childrearing practices?
How can differences in cultural beliefs between the family and professional impact the professional’s ability to support infant/toddler mental health?
How can developing “cultural competence” support the child development professional’s ability to promote infant/toddler mental health?
By Day 3:

Post your thoughts about how culture can influence childrearing practices and infant/toddler mental health, and the ways in which developing “cultural competency” can support the child development professional’s ability to promote positive outcomes for young children and their families. Include examples from your interviews.

Question Description

(1) A Scholarly Academic Journal/Article must be used pertaining to healthcare (i.e. nursing, hospital, medical care). However you could type in the search engine: “ethics case study nursing” or “ethics case study healthcare” and etc. (2) Subheadings must be included for each paragraph. Subheading should be titled: Introduction, Intervention/Solution, Possible Interventions/Solutions, Case Outcome, Cultural Issues, Ethical Principle, Conclusion. (3) Plagiarism will be checked through the software turnitin.com. (4) Please read the Rubric and Paper Instructions carefully. One of the criteria is to “Explain at least two ethical principles involved and how the principles apply to the case”

  • Explain at least two ethical principles involved (from Ethical Principles in the Ethics folder) and how the principles apply to the case

Autonomy

This principle asserts that individuals have the right to determine their own actions and the freedom to make their own decisions. Autonomous decisions are based on the individual’s values, adequate information, freedom from coercion, and reason and deliberation.

Beneficence

Beneficence is the doing of good, and it is a critical ethical principle in health care. Actions motivated by the principle of beneficence may, however, conflict with other principles, particularly patient autonomy.

Nonmaleficence

This is defined as the duty to do no harm. It is enshrined in the medical profession’s Hippocratic Oath and is likewise critical to the nursing profession. The nurse must not intentionally harm the patient. Chemotherapy is an example of a procedure that does short-term harm to the patient in hopes of producing great long-term good, and illustrates the “double effect principle.” Four conditions must be fulfilled to justify the use of the double effect principle: (1) the action must be good or at least morally indifferent; (2) the health care provider must intend only the good effects; (3) the undesired effects cannot be a means to the end or good effect; (4) there is a favorable balance between desirable and undesirable effects.

Justice

Justice requires that equals should be treated the same, and unequals should be treated differently. In the health care context, this means that patients with the same diagnosis and health care needs should receive the same care, while those with greater or lesser needs should receive care that is appropriate to their needs. Wealth, however, buys better health care regardless of the specific need of the patient. Given vast wealth disparity, it is unclear how the principle of justice can properly be applied. This problem is compounded by systemic wealth disparities that closely track racial and ethnic background, leading to racial and ethnic disparities in health care access, treatment options, and outcomes.

Fidelity

Fidelity refers to honoring one’s commitments or promises. For nurses, this specifically refers to fidelity to their patients. When a nurse is licensed, she or he accepts responsibility to respect all individuals, to uphold the Code of Ethics for Nurses, to practice within the scope of nursing practice, to keep skills current, to abide by an employer’s policies, and to keep promises to patients.

Veracity

This consists in telling the truth. Truth-telling is fundamental to the development and maintenance of trust among people. The nurse-patient relationship assumes that the nurse will deal honestly with his or her patient. Some rare instances provide narrow exceptions to the nurse general duty to tell her or his patient the truth. Deceiving a patient “for their own good” amounts to paternalism, which denies the patient’s autonomy.

Ethics Paper Instructions

2 pages Student will identify an ethical case study and develop a (3) page paper. Students should:

  • Explain at least two ethical principles involved (from Ethical Principles in the Ethics folder) and how the principles apply to the case
  • Identify several possible interventions or solutions that could have been used in this case
  • Identify and describe cultural issues that would influence the decision
  • Describe the intervention/solution that was used in this case and why it was or was not the best solution
  • Describe the outcome of the case

The paper must be in APA format (title page, running head, in-text citations, introduction, conclusion, reference page). An abstract is not required.
