Capstone Project Milestone 2:
Design for Change Proposal Guidelines
Purpose
You are
to create a Design for Change proposal inclusive of your Practice Issue and
Evidence Summary worksheet from your Capstone Project Milestone 1. Your plan is
to convince your management team of a nursing problem you have uncovered and
you feel is significant enough to change the way something is currently
practiced. In the event you are not currently working as a nurse, please use a
hypothetical clinical situation you experienced in nursing school, or nursing
education issue you identified in your nursing program.
Course
Outcomes
This assignment
enables the student to meet the following course outcomes.

CO1:
Applies the theories and principles of nursing and related disciplines to
individuals, families, aggregates, and communities from entry to the healthcare
system through long-term planning. (PO #1)

CO2:
Proposes leadership and collaboration strategies for use with consumers and
other healthcare providers in managing care and/or delegating responsibilities
for health promotion, illness prevention, health restoration and maintenance,
and rehabilitative activities. (PO #2)
Due
Date
Milestone
2 consists of the proposal for your Design for Change Capstone Project. Submit
this assignment to the Dropbox by the end of Week 4.
Points
Milestone
2 is worth 225 points.
Directions
1.
Create a proposal for your Design for Change
Capstone Project. Open the NR451_Milestone2_Design_Proposal_ template in Course
Resources to create your paper. You will include the information from Milestone
1, your practice issue and evidence summary worksheets, as you compose this
proposal. Your plan is to convince your management team of a practice problem
you have uncovered that is significant enough to change current practice.

2.
The format for this proposal
will be a paper following the Publication
manual of APA 6th edition.
3.
The paper is to be FIVE to
six-pages excluding the Title page and Reference page.
4.
As you organize your information
and evidence, include the following topics.
a.
Introduction:
Write
an introduction but do not use Introduction as a heading in accordance with
the rules put forth in the Publication manual of the American Psychological
Association(2010, p. 63). Introduce the reader to the
plan with evidence-based problem identification and solution.
b.
Change
Model Overview:Overview of the ACE Star model (the model we have been
discussing this session); define the scope of the EBP; identify the
stakeholders, and determine the responsibility of the team members.
c.
Evidence:
Conduct internal and external searches of evidence; integrate
and summarize the evidence summary worksheet from Milestone 1; develop a
recommendation for change.
d.
Translation:
develop a hypothetical action plan; include measurable
outcomes, reporting to stakeholders; identify next steps and disseminate the
findings.
e.
Conclusion:
Provide
a clear and concise summary, inclusive of the problem issue, the five
points of the ACE Star change model; and ways to maintain
the change plan.

5. Citations
and References
must be included to support the
information within each topic area. Refer to the APA manual, Chapter 7, for
examples of proper reference format. In-text citations are to be noted for all
information contained in your paper that is not your original idea or thought.
Ask yourself, How do I know this? and then cite the source. Scholarly sources
are expected, which means using peer-reviewed journals and credible websites.
6. Tables
and Figures may be added
as appropriate to the project. They should be embedded within the body of the
paper (see your APA manual for how to format and cite). Creating tables and
figures offers visual aids to the reader and enhances understanding of your
literature review and design for change.

Reflection Artifact
Please consider how you have developed the knowledge, skills, and attitudes that enable your achievement of the bachelor’s/master’s level nursing competency/ies listed below. Then write a minimum of 1000 words describing how this course has helped you achieve these outcomes.

Analyze the impact of economic, demographic and technological forces on health care delivery, and the concerns relating to ethical, legal, and social issues that influence nursing practice.

Apply a solid foundation of research and theory, focusing on the interrelationships and application of these concepts to nursing practice, education, and leadership

Improve population health across the lifespan and across the continuum of healthcare environments through health promotion and disease prevention strategies

Explore leadership and management concepts that directly and indirectly influence the nature and functioning of the healthcare system and professional nursing practice

Assess health among a diverse patient population, devising strategies for the prevention and early detection of disease across the life span

Integrate professional values, attitudes, knowledge, and behaviors into nursing practice

Part 1: Continuing Education (Part 1 should be at least 1 ½ pages and include 2 outside references plus the textbook.)

Continuing education units are required for renewal of license (in some states) and for recertification. Continuing education opens doors to job opportunities. Answer the questions:

Why is advancing your degree and knowledge important for nursing as a profession? Explain and give several reasons.

How would you respond to a nurse that states: “I only want to live and work in a state that does not require continue education credits and I never want to advance my degree?” Explain in detail what you would say and why.

Part 2: Future of Nursing (Part 2 should be at least 2 pages and include 2 outside references and the textbook.)

Review the author’s vision in the textbook (Chapter 25), of the future of nursing and complete your own research to include the Institute of Medicine’s (IOM) report.

Do you agree with the authors or do you have a different vision of nursing? Explain and provide examples as to why you do agree or do not agree.

The Interview
The medical interview serves several functions. It is used to collect information to assist in diagnosis of the present illness, to understand the patient’s values, to assess and communicate prognosis, to establish a therapeutic relationship and to reach agreement with the patient about further diagnostic procedures and therapeutic options. The interview also offers an opportunity to influence patient behaviors.
Discuss the following:  Please answer the following questions and include your rationale and evidence-based research to support your written work.
•What does it means to document accurately and appropriately?
•What are the documenting guidelines? When is it appropriate to use abbreviations? •What is the difference between subjective and objective data?
•What does it mean to demonstrate clinical reasoning skills?
•How can you use clinical reasoning to plan the organization of a comprehensive exam?
•How will you document variations of normal and abnormal assessment findings?
•What factors influence appropriate tools and tests necessary for a comprehensive assessment? •Reflect on personal strengths, limitations, beliefs, prejudices, and values.
•How will these impact your ability to collect a comprehensive health history? •How can you develop strong communication skills.
•What interviewing techniques will you use to interview the patient to elicit subjective health information about their health history?
•What relevant follow-up questions will you use to evaluate patient condition?
•How will you demonstrate empathy for patient perspectives, feelings, and sociocultural background?
•What opportunities will you take to educate the patient?

Imagine that you have been appointed the Director of Health at the Kaluyu Memorial Hospital in Nairobi, Kenya – a for-profit hospital. The facility is also a referral hospital and receives severe cases of accidents, chronicle / communicable diseases and houses an HIV / AIDS ward. As you settle into your position, you realize that the employees always act scared as they approach their superiors. Some of the employees even deliver files and leave your office in a hurry.

As you make your routine departmental visits, you realize that there are tensions between nurses and doctors, and there is a sentiment that the nurses tend to do the majority of the work within patient care but the doctors get all the credit. You realize that the employees are always looking forward to the end of their shifts to go home. Some employees even come back to work wearing the same unwashed clothes as the previous day. Too many employees are calling in sick, and many of them give weak reasons for their tardiness.

There is also a sense that employees and nurses dominate other employees in similar positions. In meetings and conference calls some employees are always quiet and never participate. You notice that people with families tend to gather and talk quietly on breaks. The new mothers working for the hospital have to use bathrooms to pump breast milk for their infants and the refrigerators do not work well. Looking at the financial statements of the hospital, you realize that the hospital expenses are higher than the industry standard, and it makes losses year after year.

Note: You may create and / or make all necessary assumptions needed for the completion of this assignment.

Write a four to six (4-6) page paper in which you:

Investigate the major demotivational factors at Kaluyu Memorial Hospital. Assess the level of impact that the identified factors could have on specific areas of service and business if the administration does not address the situation. Support your response with at least two (2) examples of the potential negative impact of the identified demotivational factors on the hospital.
Choose Maslow’s, Herzberg’s, or McClelland’s model of motivation, and argue the primary reasons why it is applicable to the Kaluyu Memorial Hospital scenario.
Formulate a communication system that will have the greatest impact on improving the situation, and specify the major reasons why your system will influence the employee behaviors for the better. Justify your response.
Use four (4) recent (within the last five [5] years) quality academic resources in this assignment.
Note: Wikipedia and other websites do not qualify as quality academic resources.

Your assignment must follow these formatting requirements:

Be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides; citations and references must follow APA or school-specific format. Check with your professor for any additional instructions.

Do you think that sentencing a person to prison is focused more on punishment or rehabilitation? Explain your reasoning.
REPLY TO MY CLASSMATE’S RESPONSE TO THE ABOVE QUESTION AND EXPLAIN WHY YOU AGREE? (A MINIMUM OF 200 WORDS EACH POST)
                                                        CLASSMATE’S POST
In my opinion, I believe sentencing someone to prison is focused more on punishment.  On one hand, I think about time.  How long does it take to truly rehabilitate someone?  What does it even mean to rehabilitate someone?  If a person enters a convenience store with a sawed-off shotgun and murders three people in the store to take the cash out of the cash register, how do you rehabilitate that type of person?  I think of rehabilitation in the form of drug/alcohol rehabilitation, for example. A person could stop drinking and taking drugs, enter treatment, attend an aftercare program, and establish a network of support that will help him or her stay off the drugs and alcohol.  But how does you rehabilitate a criminal?  In my opinion, you don’t.  It doesn’t mean that he or she, in some cases, doesn’t feel remorse for his or her actions but there is no continuing behavior moving forward like there would be in a drug and alcohol case.  So, that leads me to believe that sentencing is focused more on punishment that fits the crime.  If we take the example of the convenience store, sitting in jail for the rest of his life is probably a fitting punishment, if not the death penalty.  Rape, child molestation, and other sexually based crimes are not about changing the behavior and rehabilitation.  It’s about punishing the person for committing such heinous acts.  So, my short answer would be that sentencing is focused more on punishment.
Purpose
The purpose of this assignment is to provide the student an opportunity to reflect on the weekly concepts learned in the course.
Activity Learning Outcomes
Through this assignment, the student will demonstrate the ability to:

  1. Examine roles and competencies of advanced practice nurses essential to performing as leaders and advocates of holistic, safe, and quality care (CO1)

  3. Analyze essential skills needed to lead within the context of complex systems. (CO3)
  4. Explore the process of scholarship engagement to improve health and healthcare outcomes in various settings (CO4)

Requirements:
Reflection: write 1-2 paragraphs reflecting on your learning for the week. Guiding questions are provided or you may write about what you felt was most significant to you for the week.
Some possible areas to explore:

  • In your experience, how have changes in healthcare affected nurses’ ability to provide person-centered, holistic care?
  • As you think ahead to your future practice as an NP, which of the concepts covered this week resonates most with you? Why?
  • How do you anticipate that the concept of cultural humility will affect your practice?
