Change Implementation and Management Plan

It is one of the most cliché of clichés, but it nevertheless rings true: The only constant is change. As a nursing professional, you are no doubt aware that success in the healthcare field requires the ability to adapt to change, as the pace of change in healthcare may be without rival.
As a professional, you will be called upon to share expertise, inform, educate, and advocate. Your efforts in these areas can help lead others through change. In this Assignment, you will propose a change within your organization and present a comprehensive plan to implement the change you propose.
To Prepare:

  • Review the Resources and identify one change that you believe is called for in your organization/workplace.
    • This may be a change necessary to effectively address one or more of the issues you addressed in the Workplace Environment Assessment you submitted in Module 4. It may also be a change in response to something not addressed in your previous efforts. It may be beneficial to discuss your ideas with your organizational leadership and/or colleagues to help identify and vet these ideas.
  • Reflect on how you might implement this change and how you might communicate this change to organizational leadership.

The Assignment (5-6-minute narrated PowerPoint presentation):
Change Implementation and Management Plan
Create a narrated PowerPoint presentation of 5 or 6 slides with video that presents a comprehensive plan to implement the change you propose.
Your narrated presentation should be 5–6 minutes in length.
Your Change Implementation and Management Plan should include the following:

  • An executive summary of the issues that are currently affecting your organization/workplace (This can include the work you completed in your Workplace Environment Assessment previously submitted, if relevant.)
  • A description of the change being proposed
  • Justifications for the change, including why addressing it will have a positive impact on your organization/workplace
  • Details about the type and scope of the proposed change
  • Identification of the stakeholders impacted by the change
  • Identification of a change management team (by title/role)
  • A plan for communicating the change you propose
  • A description of risk mitigation plans you would recommend to address the risks anticipated by the change you propose

You are a staff member for a state legislator. Many issues in your state have recently become a hot topic. The legislator has formed a committee to address some of the issues brought to them by staff. The legislator’s intention is to hear policy briefs and then implement action to begin solving some of the hot topic issues in the state. You have decided to present a policy brief on an issue that you are passionate about. You will need to prepare a short presentation to present your brief to the committee. Do the following:
Review the following to help you structure your policy brief correctly:
https://www.governor.wa.gov/tags/policy-brief
https://www.seattle.gov/Documents/Departments/Council/Issues/RegulatingShortTermRentals/Regulating-Short-Term-Rentals—Policy-Brief.pdf
Part I:
Write a 500-750 word policy brief that analyzes the problem you chose specifically for your state. The policy should be concise and follow the general structure of such a policy brief. At a minimum, your policy brief should contain the following:
A title
A summary of the problem
At least 3 possible solutions
Statement and explanation of the recommended solution with rationale
The implications and benefits of the recommended policy
Brief conclusion
At least 3 sources used to support claims
Graphics and/or other visual elements so it reflects the formatting of a professional policy brief
Part II:
Create a presentation that briefly outlines the policy. Include your decision making process for presenting the information contained in the policy.
Use two to three scholarly resources to support your decision making process, some of the sources may be the same as sources used to support your recommendations in the policy brief.
You will turn in both your policy brief and presentation. See the document “submitting multiple attachments” for assistance in attaching multiple documents to the assignment dropbox.
Your presentation will require an outline of your presentation speech or speaker notes depending on the format chosen for the presentation. For PowerPoint, 8-12 slides are required with speaker notes of 50-75 words for each slide. Reference information must be provided at the end of the presentation. Reference and title slides do not count as part of the slide count.

Grant proposal in Public Health economics

Proposals must include a plan for determining the degree to which objectives are met and methods are followed. This section is extremely important as funders pay particular attention to evaluation methods, since they need help determining whether a proposed project represents an intelligent investment for them. The evaluation section should meet the following criteria:

  • Presents a plan for evaluating accomplishment of objectives
  • Presents a plan for evaluating and modifying methods over the course of the program
  • Explains who will be doing the evaluation and how they were chosen
  • Clearly states evaluation criteria
  • Describes how data will be gathered
  • Explains any test instruments or questionnaires to be used
  • Describes the process of data analysis
  • Shows how evaluation will be used for program improvements
  • Describes any evaluation reports to be produced

For this Discussion, keep these evaluation criteria in mind as you review the two grant applications listed in the Learning Resources. Two applications were submitted. Only one can receive the grant funds. Based on the information you have available, and using the evaluation rubric for each grant application, determine which one of the grants you believe should receive funding.

With these thoughts in mind:

Give three reasons why you selected the grant that you choose to fund and two reasons why you chose not to fund the other grant.  Then, briefly explain two grant-writing strategies that you might recommend to improve the unsuccessful proposal.

Discussion Assignment

The discussion assignment provides a forum for discussing relevant topics for this week based on the course competencies covered.
Post your initial response to the Discussion Area by the due date assigned.  To support your work, use your course text readings and also use outside sources. As with all assignments, cite your sources and provide references in APA format.
Start reviewing and responding to the postings of your classmates as early in the week as possible. Respond to at least two of your classmates. Participate in the discussion by providing a point of view with a rationale, challenging an aspect of the discussion, or indicating a relationship between two or more lines of reasoning in the discussion. Complete your participation for this assignment by the due date assigned.

Discussion:

Your first patient has a musculoskeletal complaint. Using the chart of musculoskeletal medical word elements from your textbook, construct 10 medical terms that would reasonably be involved in a complaint dealing with the muscles and/or bones. Each term should include its meaning in parentheses. Then list 3 tests or procedures that would reasonably be involved in evaluating a patient like this.
Your second patient has a gastrointestinal complaint. Using the chart of digestive medical word elements from your textbook, construct 10 medical terms that would reasonably be involved in a complaint dealing with the digestive tract. Each term should include its meaning in parentheses. Then list 3 tests or procedures that would reasonably be involved in evaluating a patient like this.
Your final patient has a respiratory problem, so something having to do with her breathing or lungs needs to be described. Again, use at least 10 terms from the reading, especially using the respiratory medical word elements from your textbook. Also, you need to include 3 tests or procedures that will help you to discover what the problem with your patient is.

In this course, you develop a comprehensive Course Project: Promoting Health Care

Quality. To initiate this project, this week you examine the systems and structures of an

organization with which you are familiar.

Organizational Systems and Structures, Part 1

Section 1: Organizational Systems and Structures Evaluation

When a problem related to health care quality and patient safety arises, there can be an

inclination to focus on a specific event rather than to examine the larger context

surrounding it. Analyzing the systems and structures within an organization provides a

foundation for generating a fuller understanding of how and why the event occurred and

for developing strategies to address the underlying issues. Perhaps more importantly,

such an analysis can be used to proactively identify potential challenges and improve

organizational systems and structures in order to promote positive outcomes.

In the first section of your Course Project, you analyze the systems and structures of a

health care organization with which you are familiar.

To prepare:

Identify a health care organization with which you are familiar; one that you will be

able to sufficiently analyze to complete the full scope of the Course Project. (Refer to

the Course Project Overview as needed.)

Select two or more of the following frameworks:

· Learning organizations

· Complex adaptive systems (CAS)

· Clinical microsystems

· Good to great

· The 5 Ps

With these frameworks in mind, analyze the systems and structures of the

organization. Be sure to research and consider:

· Mission, vision, and values

· Strategic plan, goals, and objectives, if possible

· Key operational processes and patterns

· Information technology use

· Organizational priorities and investments, as indicated by financial data

Week 4: Organizational Systems and Structures, Part 2

Section 1: Organizational Systems and Structures Evaluation

As you continue your evaluation of a health care organization, it is essential to pay

attention to culture. Organizational culture provides the context in which all interactions

and processes occur and is therefore central to any effort to enact change.

To prepare:

Continue to analyze your selected organization, as indicated in Week 3 (Section 1 of

the Course Project).

Continue to analyze essential elements of organizational culture and evaluate the

influence of culture on the ability to achieve goals within your selected organization.

To complete:

Write a 3- to 5-page paper in which you:

· Provide a description of the organization that you selected

· Present your analysis of the organization with attention to:

· Its mission, vision, values

· Strategic plan, goals, and objectives

· Key operational processes and patterns

· Information technology use

· Organizational priorities and investments, as indicated by financial data

· The essential elements of the organization’s culture

· The influence of culture on meeting organizational goals

Section 2: Quality Improvement Issue

To help you move forward with your Course Project, this week you identify a quality

issue that you would like to address and submit a description of this issue for Instructor

feedback.

Your proposed issue should reflect a genuine need for improvement within your

selected health care organization. Through your Course Project, you will analyze this

issue and propose strategies to improve outcome(s), including the redesign of a related

process.

To prepare:

· Review the six quality improvement aims from the Institute of Medicine (IOM)

· Safe: Avoiding harm to patients from the care that is intended to help them.

· Effective: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).

· Patient-centered: Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.

· Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.

· Efficient: Avoiding waste, including waste of equipment, supplies, ideas, and energy.

· Equitable: Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.

· Conduct a search of the literature to help you identify a specific issue that warrants attention and action to promote quality improvement.

· Consider what you have surmised thus far through your analysis of the organization

that you selected. What have you noticed that could help you identify a quality related need for improvement? For instance:

· What has captured your attention in meetings, reports, and/or daily activities?

· Have you noticed discrepancies between activities in the organization and

recommendations in the research literature, quality standards, and/or the

organization’s stated policies and procedures?

· If so, what do you think could be the reason(s) for variations or gaps between

what you have observed and what is recommended?

Identify a quality improvement issue that you would like to investigate for this project.

Note: Review the Course Project Overview/full instructions, located in the Week 3

Learning Resources, as needed to identify a quality improvement issue that is viable for

completing the full scope of this project. Once you receive a response from your

Instructor on the quality improvement issue that you submit, be sure to integrate his or

her feedback before moving forward with Section 3 of your Course Project.

To complete:

Write a brief description of the quality improvement issue that you would like to address

for your Course Project.

Assignment: State Regulations for Advanced Practice Nurses

Although your roles and responsibilities may differ based on your practice setting (think emergency department versus inpatient hospital setting or outpatient specialty medicine clinic), they may also vary depending on the state in which you practice. Because each state has its own laws and regulations that determine the scope of practice for advanced practice nurses, it is your responsibility to understand your own state’s guidelines. This Assignment involves locating the Nurse Practice Act for your state and reviewing practice guidelines, licensing requirements, and the certification process.
