The case study demonstrates a role related and ethical conundrum when the nurse is taking care of an end stage aggressive lung cancer patient, Mr. Wilson, who expressed suicidal intentions to the nurse and asked to keep the secret from his family and the hospital staff.
If the nurse knows of Mr. Wilsons’ plan for rational suicide, would the nurse be obligated to intervene? If so, what actions could the nurse take at this point? Does a nurse have the right to try to stop a person from committing rational suicide (to act in the best interest of the patient)? Is a nurse supposed to support the person’s autonomous decision to commit rational suicide? even when that decision is morally and religiously incompatible with the nurse’s perspective? What is the nurse’s role?
The case study must be typed in APA format with a minimum of 750 words (excluding first and references page) with a minimum of 4 evidence-based references using the required Arial 12 font.

Discussion question
Prepare a spreadsheet of cost savings data showing efficiency gains attributable to care coordination, and report your key findings in an executive summary, 3–4 pages in length.
The use of health information technology (HIT) has been upheld as having remarkable promise in improving the efficiency, quality, cost-effectiveness, and safety of medical care delivery in our nation’s health care system.
This assessment provides an opportunity for you to examine how utilizing HIT can positively affect the financial health of an organization, improve patient health, and create better health outcomes.
CONTEXT
Information plays a fundamental role in health care. Providers such as physicians and hospitals create and process information as they deliver care to patients. However, managing that information and using it productively poses an ongoing challenge, particularly in light of the complexity of the U.S. health care sector, with its many diverse settings for care and types of providers and services. HIT has the potential to considerably increase the productivity of the health sector by assisting providers in managing information. Furthermore, HIT can improve the quality of health care and, ultimately, the outcomes of that care for patients.
Questions to Consider
As you prepare to complete this assessment, you may want to think about other related issues to deepen your understanding or broaden your viewpoint. You are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community. Note that these questions are for your own development and exploration and do not need to be completed or submitted as part of your assessment.
· What is the main focus of information-gathering in health care?
· How can care coordination efforts enhance the collection of evidence-based data and improve quality?
· What governmental entities are leading care coordination practices?
· What influence does data analysis have on the development and advancement of health care policy?

Preparation
As the senior care coordinator in your organization, your manager has asked you to examine and report on how care coordination can generate cost savings, improve outcomes, enhance the collection of evidence-based data, and improve health care quality. She would like you to compile cost savings data in a well-organized spreadsheet and present your key findings in an executive summary.
Note: Remember that you can submit all, or a portion of, your draft spreadsheet and executive summary to Smarthinking for feedback, before you submit the final version for this assessment. However, be mindful of the turnaround time of 24–48 hours for receiving feedback, if you plan on using this free service.

Requirements

Determine how care coordination can reduce costs. Compile your cost savings data in a spreadsheet, using Microsoft Excel or suitable application of your choice. Your spreadsheet should contain at least four cost-saving elements that you will discuss in your executive summary. Identify the cost saving element, current costs, and anticipated savings.

If you elect to use an application other than Excel, check with your instructor to avoid potential file compatibility issues. Report key findings from your analysis in an executive summary.

Executive Summary Format and Length

Format your executive summary using APA style.

· Use the APA Style Paper Template, linked in the Required Resources. An APA Style Paper Tutorial is also provided (linked in the Suggested Resources) to help you in writing and formatting your executive summary. Be sure to include:

How Healthy is Your Community?

The expression “Think Globally, Act Locally” guides your Discussion this week. By accessing information from the U.S. Department of Health and Human Services, you will have the opportunity to learn how healthy your community is. You will identify the key health issues and vulnerable populations in your own county. Understanding the factors that influence the health status of residents in your county will provide you with a local perspective to compare to global health challenges you will examine in weeks to come.

To prepare for this Discussion, first learn a little about the health statistics for your county. If you are not located in the U.S., pick a state and county you might like to visit or know more about to complete this assignment. Go to the Community Health Status Indicators Report, and select your state and county to review your local report.

Q2

GRETILLAT WORKED IN FOOD SERVICE AT A NURSING HOME.

Gretillat worked in food service at a nursing home. Her duties stated that she would, at times, have to cover every aspect of operations. At times she might have to stoop, kneel, crouch, or crawl as part of needed physical movements. After 10 years, she began to suffer pain from osteoarthritis in her right knee, which made it hard to walk much. Her supervisor told her she would no longer have to make rounds at the home to reduce her walking. Three years later she had knee replacement surgery.
After the surgery, she reported a lot of pain from standing for long periods. She said she could not squat, kneel, crouch, crawl, or stand for long times. Her manager told her she could resign or be terminated since she could no longer do physical acts needed for the job. She resigned and sued for disability discrimination for failure to accommodate her disability. The district court held for the nursing home. Gretillat appealed.1. The appeals court affirmed that Gretillat’s physical impairments did not rise to the level of a disability, so she had no case. Although she was “impaired,” why was she not considered “disabled”?
2. Suppose Gretillat’s condition was severe enough to be a disability. What would the employer have needed to do then?
