As the demand for knowledge, technical savvy, compassion, and leadership increases in the field of nursing, it is important to determine what is fundamentally important to you in delivering the best nursing care. By reflecting on your values, morals, and what brought you to the profession originally, you should be able to articulate your own personal philosophy. Develop your own nursing philosophy. Consider the following questions. From your viewpoint: • What is nursing? • What is the nature of human caring? • What is the nature of nursing practice? • What is the social purpose of nursing? Explain how the philosophy you developed relates to your current or interested practice area. Format your philosophy in a way that you feel adequately conveys your ideas. You may find that writing a brief philosophy of nursing helps clarify your beliefs and values. This exercise can be done using the headings of person, environment, health, and nursing or other concepts that come to mind when you consider nursing and why you chose the nursing profession. The thought process of writing a philosophy compels you to recognize beliefs and values you hold true. Once your beliefs are clarified, surveying the definitions of person, environment, health, and nursing in the nursing theoretical works leads you to particular theoretical works to consider. Reviewing the assumptions of the philosophy, model, or theory in comparison with statements in your philosophy helps you identify an association between a theoretical work and your values and beliefs. This process may be undertaken by considering whether the concepts in the theoretical work you have selected focus on ideas similar to the values expressed in your philosophy about the patient, environment, health, and nursing. Certain framework ideas may resonate with your ideas. Chapter 4 may be helpful in this process as Pam Grace discusses values in nursing works. Guidelines for Selecting a Framework for Nursing Practice 1. Consider values and beliefs personally held about nursing. 2. Develop a philosophy statement about persons, environment, health, and nursing. 3. Survey meaning of person, environment, health, and nursing in nursing frameworks. 4. Identify two or three frameworks that resonate best with your values and definitions. 5. Review the assumptions of the frameworks you have selected. 6. Apply the frameworks in your nursing practice (try them in actual practice situations). 7. Compare the frameworks with attention to client focus, nursing action, and client outcome. 8. Review nursing literature written by persons who use the frameworks in their practice. 9. Select a framework and begin to develop its use in your nursing practice.

Research:
The temporomandibular (tem-puh-roe-man-DIB-u-lur) joint (TMJ) is the sliding hinge that connects the mandible (one on each side) to the temporal bone of the cranium (the front of the ear) (Friedman, 2019). These bones are surrounded by three extracapsular ligaments. The stylomandibular ligament is a thickened band of cervical fascia, this stretches from the styloid process and angles to the posterior border of the ramus on the mandible. This supports the weight of the jaw and the facial muscles. The lateral ligament supports the thickening of the capsule to the joint and helps to prevent the dislocation of the joint. This is connected from the zygomatic bone to the mandible. The sphenomandibular ligament is an interior ligament that is attached from the spina angularis on the sphenoid bone to the lingula of the mandible. This is the primary bone for mastication (chewing) (Jones, 2020). TMJ also has four muscles that directly affect the movement and function of the jaw. The masseter muscle, which connects from the mandible to the cheekbone, is the major muscle used for chewing. The temporalis muscle is fan-shaped and stretches from the temporal fossa and the medial face of the zygomatic process to the coronoid mandibular process and helps with chewing motions specifically for grinding and chewing food with the molars. Then two pterygoids are medial and lateral. The medial pterygoid is attached at an angle to the lateral pterygoid that forms a sling for the masseter muscle, that supports and suspends the mandible.  These two muscles help with the forward and backward motion of the lower jaw (biting, chewing, speaking, and clenching) (Bordoni, 2020). There have been recent studies (still underway) looking into botox injection for TMD relief. The long term results in animals showed decreased bone density because of the muscles being paralyzed from the botox. However, in a NY study of 79 women, 39 of whom had 2-5 rounds of botox in a year and 44 women who continued use of other treatments. They did not notice any major differences in bone density, however, they were using a very low dose that was spread out and may not work for intended purposes if TMD is severe (Communications, 2020).
Critical Thinking:
Often TMJ is confused with TMD (temporomandibular joint disorder) or TMJ syndrome, which is the condition that affects TMJ bones, muscles, and ligaments and causes pain. Although finding out exactly what causes TMD (when the joint in TMJ is not functioning properly) is extremely difficult, it’s typical symptoms are ringing ears, pain when chewing or smiling, clicking or grinding in the jaw, pain in the jaw, facial pain, and headaches (Pettit, 2017). The two basic needs we all need is to be able to hydrate so to swallow and to be able to eat. Without properly functioning jaw muscles to help chew (so there is not choking) and start the digestions process we would be a sad lot to look at. The muscles responsible for helping TMJ are not just about food though but being able to communicate or warn. I understand sign language is the alternative as well as writing things down but not being able to say words specifically because the jaw will not move to support the specific sounds could lead to some interesting word confusion.

Moreno Medical Center – Issues in Managed Care

CASE STUDY Moreno Medical Center (MMC) is a 600-bed, full-service acute care facility located in the Northwest Kelsey community. The hospital is a physician-owned, for-profit hospital that takes pride in the provider-patient relationship. With recent changes in managed care, every dollar, process, and interest of the patient serves as a common ground for making an organization more efficient.   To remain competitive in the marketplace, the management team at MMC decided to take a look at various processes within the hospital and noted that the overall patient experience in the outpatient surgical clinic was less than satisfactory. The main problem seems to focus on long delays throughout the entire admission to discharge process. If the hospital wants to maintain its competitive position, the administration will have to redesign the current process to improve patient satisfaction.   The hospital administration appointed an internal quality consultant to facilitate the planning process. The quality consultant for this new organizational role is an experienced intensive care RN with a business degree. She understands that health care is a personal encounter and that patients and families expect to have their priority needs met when experiencing a health care issue.   The quality consultant decided to work closely with the surgical department RN administrator who would be a powerful change agent in leading the improvement process.
