Key Topics in Health Care Technology Evaluation The amount of new information and data, and the number of available technologies are growing at an ever-accelerating rate. Did you know that during any given 24 hours, humanity generates enough new information to fill the Library of Congress 70 times (Smolan & Erwitt, 2012)? As a nurse informaticist, it is important to keep current on new developments in the field, but with the rapid pace of change, that effort can be overwhelming. It is easier to keep current with key trends if nurse informaticists focus on selected issues. In this Discussion, you consider key topics in the field of health care technology. You then consider the different approaches you could take when designing an evaluation in these areas. For example, if you are interested in usability, your goal could be to determine if a system is user friendly from the viewpoint of a nurse. A different goal might be to determine if the location of the system facilitates ease of use from the viewpoint of physicians. Note: This Discussion serves as practice for the first part of your Evaluation Project. What you derive from your Discussion with colleagues will likely inform the work that you do in Part 1 of the Evaluation Project. The Discussion focuses on the following major topics in the health care information field: Implementing HIT Systems

  • Consumer health information
  • Computerized Physician Order Entry (CPOE)
  • Decision support systems
  • Electronic health records (EHR)
  • Tele-medicine and eHealth
  • Nursing documentation
Other Issues Related to the Use of HIT Systems
  • Interoperability
  • Unforeseen consequences
  • Usability
To prepare:
  • Select at least two topics from the lists above that are relevant to your current organization or that are of particular interest to you. Read the articles in this week’s Learning Resources that relate to these topics. Consider why these topics are of interest to you, what relevance they have to health care organizations, and how they impact your professional responsibilities. Choose one topic to be the focus of your Evaluation Project, and consider potential evaluation goals.
  • Determine the viewpoint from which you would approach the evaluation, and why.
By tomorrow, post a minimum of 550 words essay in APA format with a minimum of 3 references from the list of required resources below, that addresses the level one headings as numbered below: 1)      Post the two topics you identified as most relevant to your organization or to you personally, and explain why you selected those topics.   2)      Identify the topic you selected for your Evaluation Project, and propose three potential evaluation goals for this topic.   3)      Identify the viewpoint you would use with each goal, and explain why.     Required Readings       Friedman, C. Q1. Define fiscal policy and its key objectives. What government agencies are responsible for making decisions on fiscal policy actions and implementations?
Q2. Describe briefly the short-run and long-run effects of the rising budget deficit and public debt. What is the crowding out effect? Why has the US economy seldom been faced with the crowding out effect in recent decades despite the skyrocketing budget deficit and public debt?
Q3. Explain why the velocity of either M 1 or M 2 is expected to rise during a rising interest rate trend and why it is expected to fall during a declining interest rate trend?
Q4. Why do neither monetarists nor Keynesians suggest maintaining a constant federal funds rate target? What potential consequences do they anticipate if a constant target were to be maintained?
Q5. Why does excessive money supply, usually called printing money, accelerate inflation? Give an example.

  • Explain the positive and negative aspects of these forces that act against each other, in case of government healthcare programs such as Medicare and Medicaid.
  • How do these two forces play a part in the development of healthcare policy relative to government healthcare programs?
  • All government programs are subject to legislative actions and administrative implementation. Based on the stages through which each item of public policy passes, answer the following questions:
    • How can a government healthcare program that was first envisioned be the same program when implemented?
    • Why do public policy and legislative action have such a significant impact on the implementation of government healthcare programs?
State health programs are typically funded in part by federal grants. The federal government pays a percentage of the costs and the states pay for the remaining balance. Based on your understanding of the above statement, answer the following:
    • Discuss the power and discretion states have in making decisions for federally funded health programs.
    • Using the PPACA as an example, what role will states play in implementing the policy?
    • Describe the level of financial support expected from the federal government.

All Punnett Squares must be shown for deriving the answer.
Turn in the problems, worked on this paper or an attached sheet. Show your work!!!
    1. In humans, the allele for albinism (a) is recessive to the allele for normal pigmentation (A).
A normally pigmented woman whose father is an albino marries an albino man whose parents are normal. They have three children, two normal and one albino.
Give the genotypes for each person in the above scenario. Use the Punnett Square to prove your answer.
    Genotype
    The woman _____________
    Her Father ______________
    The albino man __________
    His Mother _____________
    His Father ______________
    3 children ____, ____, ____
Incomplete Dominance is an intermediate inheritance in which one allele for a specific trait is not completely dominant over the other allele. Each allele is expressed in the heterozygous genotype producing an intermediate phenotype.
For example, in certain snapdragon flowers, the gene R codes for red flowers while its allele R’ codes for white flowers. The RR genotype produces red flowers, R’R’ produces white and heterozygous RR’ produces pink flowers.

Codominance occurs when two alleles are equally dominant, it is characterized by full expression of both alleles and both appear in a phenotype. The roan cow is the result of a heterozygous expression of colored and white hairs from the parents.
    2. In four o’clock flowers the gene R codes for red flowers and its allele R’ codes for white flowers. The genotype RR produce red flowers R’R’ results in white and the heterozygous condition RR’ produces pink flowers. What type of dominance is this? Give the genotypic and phenotypic ratios of the offspring of a cross between a white and pink flowering four o’clock.
    3. Give the genotypic and phenotypic ratios of the offspring of a cross between two pink flowering four o’clock plants.
    4. In cocker spaniels, black coat color (B) is dominant over red (b), and solid color (S) is dominant over
spotted (s). If BbSs were crossed with Bbss, give the phenotype ratio for the offspring of this dihybrid cross. Show your work.
MULTIPLE ALLELES INHERITANCE – Characteristics determined by Multiple Allele Inheritance are determined by more than two alleles of the same gene in the general population, although any one individual only has a single pair of genes (2 alleles) for the characteristic.
For example ABO blood groupings are determined by three alleles of the same gene. The allele IA produce a type A antigen on the surface of the red blood cell, IB produces the type B antigen and the allele i does not code for any antigen. The genotypes IAIA and IAi produce type A blood, IBIB and IBi produce type B, IAIB produces type AB and ii produces type O blood. Remember that in ABO blood groupings the alleles for type A and type B are codominant. IA and IB are co-dominant genes so that the combination IA IB produces the AB blood type.
    5. Knowing what you do about the ABO blood types (M ultiple Allele Inheritance ). .
Can each of these parents produce the given child? Show your work for each cross.
    Parent Child Yes/No
    a) A x AB B ______
    b) A x O A ______
    c) A x B O ______
    d) A x A AB ______
    e) B x B O ______
    f) AB x AB A ______
X-LINKED INHERITANCE – The genes for X-linked characteristics are located on the X chromosome only,
thus females have two genes for each X-linked characteristic while males have only one.
As a result X-linked recessive characteristics appear more often in males than females.
In cats coat color is determined by one pair of X-linked alleles. The gene XB codes for black coat while its allele Xb produces yellow coat. In females the heterozygous condition XBXb results in a calico coat (female).
    6. A calico cat (female) is mated with a yellow cat. Give the genotypic and the phenotypic ratios for the expected offspring. The interaction between XB and Xb to produce calico is an example of ____ inheritance.
    6. (2 points) A yellow female cat is mated with a black male. Give the genotypic and phenotypic ratios for the offspring
    Answer this following question.
    1. Observe an ice cube.  This is water in a solid form, so it has a high structural order.  This means that the molecules cannot move very much and are in a fixed position.  The temperature of the ice is zero degrees celcius.  As a result, the entropy of the system is low.
    2. Allow the ice to melt at room temperature.  What is the state of molecules in the liquid water now?  How did the energy transfer take place?  Is the entropy of the system higher or lower?  Why?
    3. If you were to heat the melted water to its boiling point, what would happen to the entropy of the system?
    4. Describe the structure and complementary base pairing of DNA.
    5. Explain how single nucleotide changes can have vastly different effects on protein function.
    6. In the past 25 years, we have learned a lot about DNA, and are now able to manipulate genes.  Plants are genetically modified to possess desirable traits such as resistance to disease and to grow with less water and fertilizer.  There are even certain Idaho potatoes that all grow to the same size, so McDonald’s french fries are the same length!  Human genes are inserted into bacteria to inexpensively produce drugs that treat diseases.  Soon, non-life threatening cosmetic changes will be available for those who can afford them.Conduct an internet search to find an interesting example of genetic engineering.  Then, summarize what you discovered.  Next, respond to at least one other student’s findings.
    Answer:
    1.
    2.
    3.
    4.
    5.
    6.

    NURSING APA with REFERENCES
    Essay Assignment:
Answer all of the following questions using headers to separate topics.

    1. As one learns about all of the wonders of modern technologies, how would you respond (positive or negative, and why) to Dr. Victoria Sweet who wrote: In trying to get control of healthcare costs by emphasizing “efficiency,” we’ve headed down a wrong path. Medicine works best—that is, arrives at the right diagnosis and the right treatment for the least cost—when the doctor has enough time to do a good job, and pays attention not only to the patient but to what’s around the patient. Dr. Sweet calls this approach Slow Medicine, and she believes that, put into wider practice, it would be not only more satisfying for patient and doctor, but also less expensive.
    2. Consider the involvement of insurance companies, the big business of healthcare, and cost-cutting activities. Explain what you believe is meant by a cardiac patient who, upon meeting his surgeon for the first time prior to surgery, says to the surgeon, “Hello, sir. I’m Mr. John Smith, husband of Mary Smith and father of Jeanette Smith. I’m a carpenter who has built close to 80 new homes over the last 20 years with these two hands. I’m not the heart attack in Room 403.” and why.
    3. It’s been said that true quality improvement is only possible when those involved in the process can be completely honest with one another. In the case of healthcare, when mistakes cause serious pain, suffering, and possibly death, how is true quality improvement possible? When leveraging 6 Sigma, is total honesty really necessary? Explain.
    4. Everyone agrees that reading the Electronic Healthcare Records is much easier than the hand-written entries of old, but many complain that too many are focusing too much on the computer screen instead of on the human right in front of them. Is this just lack of skill or training? Is it poor computer system design? Is it just the nature of the transition from the old way to the new? Is patient care suffering because computers and all the displays, alerts, and alarms distract us?

Discussion Not An Essay/Please Follow Directions/ Please Use Decision Tree For Statistics

What does it mean to analyze the collected data from a program evaluation? An analysis involves reviewing multiple components, such as analyzing and synthesizing findings. This includes tabulating information to explore how many individuals participate in the program, reviewing demographics, analyzing the number of participants achieving the outcome goals, and analyzing and stratifying data by race, gender, age, income level, and geographic locations. It is also important to conduct a comparative analysis to review strengths, weaknesses, and any limitations and to review gaps. The next step is combining all of this information to make decisions and present to stakeholders for further input and suggestions regarding the program.
